Carga alostática y rendimiento cognitivo en el trastorno depresivo mayor

Abstract

El estrés sostenido o repetido se ha asociado con alterar la homeostasis del organismo, y consecuentemente, predisponerlo con una vulnerabilidad para los trastornos afectivos, los déficits cognitivos y a otras condiciones médicas, como la obesidad o el riesgo cardiovascular. Clínicamente, se puede medir el impacto del estrés en el organismo mediante un índice de carga alostática que engloba medidas metabólicas, cardiovasculares, inmunológicas y endocrinológicas. En el presente estudio analizamos la asociación entre la carga alostática y el rendimiento cognitivo en pacientes con TDM (n=38) y controles sanos (n=36) reclutados en el Servicio de Psiquiatría del Hospital Universitari de Bellvitge (HUB). Para ello, se ha construido un índice de carga alostática compuesto por 15 biomarcadores y se ha estudiado su relación con el rendimiento neuropsicológico en tareas de memoria de trabajo visoespacial, evaluados mediante la batería Cambridge Neuropsychological Test Automated Battery (CANTAB). El índice de carga alostática calculado se relacionó con las tareas cognitivas en nuestra muestra de estudio, sin embargo, no pudo predecir el desempeño cognitivo en la regresión. Cabe destacar que las variables edad, años de educación y uso de benzodiacepinas se asociaron significativamente con el desempeño cognitivo. Por lo tanto, asumimos que el índice de carga alostática calculado no predice per sé el funcionamiento cognitivo en la muestra analizada, sin embargo, cabe destacar la relevancia de determinadas variables sociodemográficas y clínicas en la asociación con el rendimiento de la memoria de trabajo visoespacial.
