Abstract

RESUMEN : Introducción: Las complicaciones pasadas el día 100 postrasplante, conocidas como complicaciones tardías, son eventos poco descritos en el trasplante tipo haploidéntico, en el que donante y receptor solo comparten un haplotipo HLA. Dentro de las complicaciones tardías postrasplante haploidéntico podemos encontrar dos tipos, las infecciosas (bacteriemias, neumonía, infecciones por CMV, infecciones fúngicas, etc) y las no infecciosas (EICH crónica, neumonitis, etc). El objetivo de nuestro estudio es analizar de manera descriptiva estos dos grupos de complicaciones tardías en el haplotrasplante. Metodología: Revisión sistemática de historias de los pacientes que recibieron un trasplante haploidéntico entre 2012 y 2018 en el Hospital Marqués de Valdecilla. De los 60 pacientes se describen las complicaciones tardías de los supervivientes al día 100 postrasplante (n= 47). Resultados: En cuanto a las complicaciones tardías, una de las mas relevantes es la enfermedad injerto contra huésped crónica que se presenta en el 57,4% (27) de nuestro pacientes, presentándose en su forma moderada y grave en 10 (21,3%) y 3 (6,4%) pacientes. En cuanto a las complicaciones no infecciosas las más relevantes han sido la presencia de neuropatía en 14 (29,8%) pacientes y algún tipo de patología pulmonar en 10 (21,3%). En las complicaciones infecciosas destacamos la presencia de algún episodio neumónico en el 44,7% (21), diagnóstico de sepsis en 9 (19,1%) pacientes e infección por CMV en 11 (23,4%) pacientes. La mediana de seguimiento de nuestra serie (n= 47) fue de 38,5 meses con una supervivencia al año del 87,2% y a los 2 años del 74,0%, con 13 pacientes fallecidos. Por último, 14 pacientes (29,8%) recaen, siendo la recaída la principal causa de muerte 19,1% (9). Conclusiones: En nuestro estudio la EICH crónica es la complicación no infecciosa más relevante y aparece en un 57,4% y la neumonía asociada o no a sepsis es la más frecuente (44,7%) de las complicaciones infecciosas. Sin embargo, la mortalidad relacionada con el trasplante es baja de 8,5% (4). A pesar del número de complicaciones tardías, la supervivencia de los pacientes que sobreviven más allá de los 100 primeros días del trasplante es elevada, con un 74% de los pacientes vivos a los 2 años del trasplante.

ABSTRACT : Introduction: Complications which may arise once a hundred days have passed, known as late complications, are barely described in haploidentical transplantation where both the donor and the recipient just share a HLA haplotype. Among late haploidentical complications appearing after a transplant, we may find two main types: infectious (bacteremia, pneumonia, CMV infections, fungal infections, etc.) and non-infectious (chronic GvHD, pneumonitis, etc.). The main aim of our research is to analyse in depth these two groups of late effects in haplotransplantation. Methodology: Systematic review of medical records from patients who received a haploidentical transplantation between 2012 and 2018 at Marqués de Valdecilla hospital. We have studied late complications in 60 different patients who have survived at day 100 post-transplant (n=47). Results: As for late complications, one of the most relevant is this of chronic graftversus-host disease (GvHDc) that occurs in 57.4% (27) of our patients, 21,3% (10) of them in a moderate way and 6,4% (3) of them in a severe one. Regarding noninfectious complications, the most relevant has been the presence of neuropathy in 14 (29,8%) patients and some type of pulmonary pathology in 10 of them. In terms of infectious complications, it should be highlighted the presence of a pneumonic episode in 44.7% (21), sepsis diagnosis in 9 (19,1%) patients and CMV infeccion in 11 (23,4%) of them. The mean follow-up was 38.5 months with a 87,2% one-year survival and a 74,0% regarding 2-year survival, with 13 deceased patients. In addition, 14 patients suffered a relapse episode, being primary disease considered as the main cause in 9 out of 13 patients. Conclusions: In our study, chronic GVHD is the most relevant non-infectious complication and appears in 57.4%, and pneumonia associated or not with sepsis is the most frequent (44.7%) of infectious complications. However, transplant-related mortality is low at 8.5% (4). Despite the number of late complications, the survival of patients who survive beyond the first 100 days of transplantation is high, with 74% of patients alive 2 years after transplantation.
