Abstract

RESUMEN : La terapia antitumoral está siendo uno de los campos de investigación con perspectiva más prometedora de los últimos años, llegando a su punto álgido con la consecución del Nobel de Medicina en 2018, gracias al descubrimiento del importante papel de los inhibidores de puntos de control inmunitario (immune checkpoints inhibitors o ICIs) que consiguen liberar los frenos de la respuesta inmune y relanzar al sistema inmunitario para atacar el cancer. No obstante, en este campo que avanza y crece cada vez más rápido, despuntan ahora otras estrategias de inmunoterapia de base celular. En este sentido, algunos de los avances más importantes de los últimos años están siendo las terapias con células T de receptor quimérico (CAR-T cells), que han supuesto una auténtica revolución en el tratamiento de las neoplasias hematológicas. En este trabajo, se analizan las interacciones entre el sistema inmunitario y el tumor, el papel de los inmune check points y de sus inhibidores (los ICIs), así como las diferentes terapias de base celular, en especial las CAR-T cells. Se profundiza en los protocolos ya aprobados, así como en los desafíos a los que se enfrentan estas nuevas terapias.

ABSTRACT : Anti-tumoral inmunotherapy is being one of the most developed and promising pathways of investigation in the recent years, reaching its peak with the 2018 Nobel prize in Medicine award thanks to the discovery of the important role of immune checkpoint inhibitors or ICIs when it comes to releasing the brakes of the immune response and thereby unleashing our immune cells to attack tumors. However, in this rapidly advancing and growing field, cell-based immunotherapy strategies are now emerging. In this sense, one of the most important advances in recent years has been the chimeric receptor T-cells (CAR-T cells) therapies, which have brought a real revolution in the treatment of hematological malignancies. In this work, we discuss the interactions between the immune system and cancer, the role of immune checkpoints and their inhibitors, as well as the different cell-based therapies, specifying even more in CAR-T cells. Digging deeper into current approvals as well as the challenges these new therapies face.
