Abstract

RESUMEN: Introducción y objetivos. Los pacientes con EPOC tienen una mayor prevalencia de osteoporosis que la población general, y con ello mayor riesgo de fracturas. El objetivo de este estudio fue determinar la prevalencia real de fracturas vertebrales y de osteoporosis en este grupo de pacientes, así como valorar si la presencia de estas fracturas tenía una repercusión negativa sobre la EPOC. Material y métodos. Estudio descriptivo retrospectivo de los pacientes ingresados en el HUMV con diagnóstico de EPOC entre julio y diciembre de 2017. Se recogieron datos demográficos y clínicos previos, y se revisaron todos los procesos que habían sufrido estos pacientes desde el momento de su inclusión en el estudio hasta el fallecimiento o el momento de recogida de los datos. Resultados. Se incluyeron 264 pacientes (73,9% varones; media de edad 76,01 años). Tenían diagnóstico de osteoporosis el 17%, diagnóstico de fractura vertebral el 12,9% y fracturas vertebrales no diagnosticadas el 32,6%. Los pacientes con fractura vertebral presentaron más ingresos y mayor mortalidad. Conclusiones. Encontramos que existe un infradiagnóstico e infratratamiento de fracturas vertebrales y osteoporosis en pacientes ingresados por descompensación de EPOC. Los pacientes con fracturas vertebrales presentaron exacerbaciones más graves y mayor mortalidad.

ABSTRACT: Introduction and objectives. COPD patients have higher prevalence of osteoporosis than general population, so they have an increased risk of fracture. The aim of this study was to determine the real prevalence of vertebral fractures and osteoporosis in this group of patients, as well as to assess if vertebral fractures caused any harmful impact on COPD. Material and methods. Retrospective descriptive study of patients admitted to the HUMV with diagnosis of COPD between July and December 2017. Previous demographic and clinical data were collected, and all the processes that these patients had suffered from the time of their inclusion in the study until their death or the moment of data collection. Results. 264 patients were included (73,9% males; mean age 76,01 years). 17% of them were diagnosed of osteoporosis, 12,9% of them were diagnosed of vertebral fracture and 32,6% had undiagnosed vertebral fractures. Patients with vertebral fracture had more hospital admissions and higher mortality. Conclusions. We found that there is an underdiagnosis and undertreatment of vertebral fractures and osteoporosis in patients admitted for decompensation of COPD. Patients with vertebral fractures had more severe exacerbations and increased mortality.
