Abstract

[ES]: La Enfermedad de Parkinson (EP) se caracteriza clínicamente, además de por sus síntomas motores clásicos, por otros no motores como los psicóticos, que afectan gravemente a su calidad de vida aumentando la morbimortalidad. La Psicosis asociada a la Enfermedad de Parkinson (PEP) tiene una prevalencia muy alta (aproximadamente la mitad de estos pacientes lo presenta durante su vida), siendo lo más común las alucinaciones visuales. No se conoce con exactitud la etiología, pero se piensa que puede deberse al uso de fármacos antiparkinsonianos, a la presencia de enfermedad por cuerpos de Lewy subyacente, o a una combinación de ambos. El principal problema está en el tratamiento, debido al mecanismo opuesto entre los antiparkinsonianos (dopaminérgicos) y antipsicóticos (antidopaminérgicos), por lo que hay que tratar de utilizar antipsicóticos que no empeoren la función motora que ya se encuentra deteriorada. La base del tratamiento consiste en reducir los antiparkinsonianos cuando sea posible y utilizar antipsicóticos atípicos, que presentan menos efectos extrapiramidales que los típicos. Según la evidencia científica, la clozapina es el mejor antipsicótico, pero debido al estrecho control que requiere por sus graves efectos adversos, el más utilizado en la práctica clínica es la quetiapina.

[EN]: Parkinson's disease (PD) is clinically characterized, in addition to its classic motor symptoms, by other non-motors such as psychotic symptoms, which severely affect their quality of life increasing morbimortality. Parkinson's Disease Psychosis (PDP) has a very high prevalence (approximately half of these patients present it during their lifetime), being visual hallucinations the most common. The etiology is not exactly known, but it is thought to be due to the use of antiparkinsonian drugs, the presence of underlying Lewy body disease, or a combination of both. The main problem is the treatment, due to the opposite mechanism between antiparkinsonian (dopaminergic) and antipsychotic (antidopaminergic), so it is necessary to try to use antipsychotics that do not worsen motor function that is already deteriorated. The basis of treatment is to reduce antiparkinsonians when possible and to use atypical antipsychotics, which have less extrapyramidal effects than typical ones. According to scientific evidence, clozapine is the best antipsychotic, but because of the narrow control it requires due to its serious adverse effects, quetiapine is the most widely used in clinical practice.
