Abstract

La epilepsia es una entidad clínica caracterizada por la recurrencia de crisis epilépticas. Más de 60 millones de personas en el mundo se encuentran diagnosticados de esta enfermedad, y hasta el 30% de estos pacientes presenta lo que se denomina una epilepsia refractaria (definiéndose ésta como aquella que no responde adecuadamente a dos o más pautas de fármacos antiepilépticos). Estos pacientes requieren de un estudio exhaustivo para determinar si padecen o no realmente epilepsia (con frecuencia otros trastornos paroxísticos como los síncopes son catalogados erróneamente como epilepsia) y de igual modo para establecer cuál es el tratamiento más apropiado para su enfermedad. En la actualidad existen diversas líneas de tratamiento para los pacientes que padecen epilepsia farmacorresistente, entre las que se incluyen la cirugía (donde destaca la resección cortical focal), las terapias de estimulación nerviosa como la Estimulación del Nervio Vago o las terapias dietéticas, siendo la más frecuentemente empleada la dieta cetogénica, de especial interés en la edad pediátrica. En este trabajo se revisará la información concerniente a las indicaciones de cada una de estas terapias, así como su mecanismo de acción, su eficacia y los efectos adversos más relevantes derivados de su uso.

Epilepsy is a clinical entity characterised by the recurrence of epileptic seizures. More than 60 million people in the world are diagnosed with this disease, and up to 30% of these patients have what is called refractory epilepsy (defined as one that does not respond adequately to two or more antiepileptic drug regimens). These patients require a thorough study to determine whether or not they actually have epilepsy (often other paroxysmal disorders such as syncopes are erroneously labeled as epilepsy) and also to determine the most appropriate treatment for their disease. There are currently several treatment lines for patients suffering from drug-resistant epilepsy, including surgery, nerve stimulation therapies such as Vagus Nerve Stimulation or dietary therapies, the most frequently used being the ketogenic diet, of special interest in the pediatric age. This review will summarize the information concerning the indications of each of these therapies, as well as its mechanism of action, its efficacy and the most relevant adverse effects derived from its use.
