Abstract

Antecedentes: Los gliomas difusos son los tumores primarios más comunes en el sistema nervioso central, con una baja tasa de supervivencia. La biopsia tumoral es el método actual para diagnosticarlos, pero presenta riesgos y no siempre representa todas las características moleculares del tumor. Se propone la biopsia líquida como una alternativa menos invasiva y más precisa, permitiendo una imagen longitudinal del tumor durante su evolución. Aunque la biopsia tumoral sigue siendo el estándar, los estudios comparan los hallazgos de la biopsia líquida con la biopsia tumoral. El objetivo de esta revisión es describir los conocimientos actuales sobre la biopsia líquida, sus ventajas y limitaciones. Métodos: Se realizó una búsqueda en las bases Pubmed, Web of science y Scopus de las cuales se extrajeron 144 artículos, cribados por dos revisores y analizados por Quadas-2, herramienta para evaluar la calidad de los estudios y por la clasificación de niveles de evidencia de Oxford. Resultados: En la mayoría de los estudios se encontró una concordancia mayor al 60% de ctDNA entre LCR y la biopsia tumoral, lo que proporciona una nueva expectativa y avance en el diagnóstico molecular de los gliomas difusos. Conclusión: Esta técnica es prometedora en pacientes con tumores de difícil acceso para biopsia. Proporciona información molecular longitudinal, aborda la heterogeneidad intratumoral y ayuda a diferenciar recidivas de pseudoprogresión. Sin embargo, se requieren más estudios y estandarización para su uso generalizado.

La biòpsia tumoral és el mètode actual per demostrar-los, però presenta riscos i no sempre representa totes les característiques moleculars del tumor. Es proposa la biòpsia líquida com una alternativa menys invasiva i més precisa, i permet una imatge longitudinal del tumor durant la seva evolució. Tot i que la biòpsia tumoral continua sent l'estàndard, els estudis comparen els resultats de la biòpsia líquida amb la biòpsia tumoral. L'objectiu d'aquesta revisió és descriure els coneixements actuals sobre la biòpsia líquida, els avantatges i les limitacions. Mètodes: Es va realitzar una cerca a les bases Pubmed, Web of science i Scopus de les quals es van extreure 144 articles, garbellats per dos revisors i analitzats per Quadas-2, eina per avaluar la qualitat dels estudis i per a la classificació de nivells de Evidència d'Oxford. Resultats: En la majoria dels estudis es va trobar una concordança major al 60% de ctDNA entre LCR i la biòpsia tumoral, cosa que proporciona una nova expectativa i avenç en el diagnòstic molecular dels gliomes difusos. Conclusió: Aquesta tècnica és prometedora en pacients amb tumors de difícil accés per a biòpsia. Proporciona informació molecular longitudinal, aborda l'heterogeneïtat intratumoral i ajuda a diferenciar recidives de pseudoprogressió. No obstant això, calen més estudis i estandardització per al seu ús generalitzat.

Background: Diffuse gliomas are the most common primary tumors in the central nervous system, with a low survival rate. Tumor biopsy is the current method to demonstrate them, but it carries risks and does not always represent all the molecular characteristics of the tumor. Liquid biopsy is proposed as a less invasive and more precise alternative, allowing a longitudinal image of the tumor during its evolution. Although tumor biopsy remains the standard, studies compare the results of liquid biopsy with tumor biopsy. The objective of this review is to describe the current knowledge about liquid biopsy, its advantages, and limitations. Methods: A search was carried out in the Pubmed, Web of Science and Scopus databases, from which 144 articles were extracted, screened by two reviewers, and analyzed by Quadas- 2, a tool to assess the quality of the studies and by the classification of levels of Oxford evidence. Results: In most of the studies, a concordance greater than 60% of ctDNA was found between the CSF and the tumor biopsy, which provides a new expectation and advance in the molecular diagnosis of diffuse gliomas. Conclusion: This technique is promising in patients with tumors difficult to access for biopsy. It provides longitudinal molecular information, addresses intratumoral heterogeneity, and helps differentiate recurrence from pseudoprogression. However, further studies and standardization are required for its widespread use.
