Migrene - årsaksmekanismer : En litteraturstudie

Abstract

Migrene er en vanlig form for hodepine, og den globale prevalensen av migrene er 10%. Det patofysiologiske grunnlaget for migrene er ikke fullt ut forstått, og opp gjennom historien har vaskulære mekanismer fått betydelig oppmerksomhet. Denne litteraturstudien drøfter mulige årsaker til migrene, både hodepine og aura, og om det er noen felles underliggende årsak til disse. Migreneaura kjennetegnes av visuelle og/eller sensoriske symptomer, eller afasi/dysfasi som oppstår før hodepinen oppstår. Det er en bred aksept for at aura er forårsaket av cortikal spreading depression (CSD), men hva som fører til CSD er ikke etablert. Hypoglykemi, hypoksi og KCl-infusjon kan utløse CSD, og legemidler som hemmer CSD synes å hindre migreneaura. Migrene med aura er assosiert med kardio-og cerebrovaskulære lidelser, slik som CADASIL. Aktivering av det trigeminovaskulære system er en sentral del av patofysiologien bak migrenehodepine, men den primære årsaken til denne aktiveringen er ufullstendig forstått. Andre viktige elementer i patofysiologien bak migrenehodepine er neuropeptider, sensitivisering, serotoninmetabolisme og genetikk. Familiær hemiplegisk migrene kan skyldes mutasjoner i flere gener, men det ser ut til at disse mutasjonene ikke spiller en viktig rolle for utvikling av vanlig migrene. CSD er en grunnleggende mekanisme ved migreneaura, men det er høyst usikkert om fenomenet er nødvendig for at hodepinen oppstår.
