Efecto del calostro en prematuros con bajo peso al nacer

Abstract

Recientemente se ha observado un aumento en el riesgo de morbimortalidad en relación con una menor edad gestacional del recién nacido. La leche materna tiene componentes que refuerzan el sistema inmunológico e influyen en el crecimiento. Sin embargo, casi el 50% de los hospitales españoles utilizan leche de fórmula en prematuros relacionándose esta alimentación con episodios diarreicos hasta en el 80% de los lactantes. Esta revisión sistemática tuvo como objetivo explorar si la administración de calostro materno en la mucosa oral del bebé prematuro de menos de 1500 gramos conducía a una reducción en la morbimortalidad y a la mejora en el recién nacido. Los estudios utilizados fueron ensayos controlados y estudios piloto aleatorios y ensayos controlados no aleatorios. Se incluyeron a los prematuros con bajo peso al nacer (menores de 1500 g), y se compararon con placebo o atención estándar. La intervención fue realizada hasta que cumplieran 32 SG o mínimo durante las primeras 46 horas hasta un período de quince días de vida. No se incluyó a los ensayos que utilizaron calostro animal y a los neonatos a término. En PubMed se usaron los términos MeSH: ("Colostrum"[Mesh]) AND ("Infant, Low Birth Weight"[Mesh]), y, en CINAHL, se utilizó la estrategia de búsqueda: (“Colostrum” AND “Infant, Low Birth Weight”). No se encontraron resultados significativos en cuanto sepsis de inicio, incidencia de enterocolitis necrotizante y morbimortalidad. En algunos estudios se pudo observar menor incidencia de neumonía asociada al respirador, mayor porcentaje de peso diario, mayores niveles medios de IgA sérica y lactoferrina, y menor duración hospitalaria, en los prematuros alimentados con calostro. Sin embargo, son necesarios más ensayos controlados aleatorios adecuados para abordar este tema.
