Abstract

La demencia frontotemporal (DFT) segunda forma de demencia de inicio temprano tras la enfermedad de Alzheimer (EA) representa un grupo heterogéneo de trastornos que en los últimos años está experimentando extraordinarios avances en su conocimiento fisiopatológico. La identificación de características clínicas puede ayudar en la predicción de su patología molecular o de una determinada alteración genética, y ser objetivo de nuevos ensayos con fármacos dirigidos hacia dianas concretas.En este trabajo se ha realizado la caracterización fenotipo-genotipo de una cohorte de pacientes con sospecha clínica de DFT, llevándose a cabo un estudio multidisciplinar en el que se han analizado características demográficas, variabilidad clínica, conductual, neuropsicológica, anatomopatológica y genética, mediante la detección de mutaciones en los genes: MAPT, GRN, FUS, VCP, TARDBP,CHMP2By C9orf72.Se han tomado muestras de sangre de 74 pacientes y utilizando técnicas de secuenciación convencional se han detectado expansiones en el gen C9orf72 en 6 de estos pacientes y mediante la llamada Next Generation Sequencing (NGS) mutaciones en 6 pacientes en GRN, dos de ellas de significado clínico desconocido, 0 en MAPT, 2 pacientes con mutaciones en VCP y 3 en FUS, completándose el análisis en este caso con estudios de expresión del gen.DFT puede confundirse clínicamente con otras entidades como trastornos psiquiátricos y EA (22% de nuestras necropsias). Memoria y fluencia verbal son los dominios cognitivos más frecuentemente afectados en nuestros pacientes. El uso de biomarcadores resulta primordial para lograr más precisión diagnóstica. NGS abre una puerta al descubrimiento de nuevas mutaciones sin embargo el manejo e interpretación de las variantes de significado incierto supone un proceso complejo.
