Inyecciones intravítreas de Dexametasona en edema macular diabético

Abstract

[ES] Los objetivos principales del trabajo son medir la respuesta anatómica y funcional en pacientes tratados con Dexametasona intravítrea en edema macular diabético y analizar la incidencia de complicaciones. Se trata de un estudio observacional retrospectivo en el que se han incluido pacientes con edema macular diabético tratados en el Hospital Universitario de estos últimos años con implantes de Dexametasona intravítreo. Las principales variables de resultado han sido: a) la agudeza visual obtenida en cada inyección, b) la agudeza visual final y c) el grosor del edema tras cada inyección. Como complicaciones se han analizado las cataratas, glaucoma y endoftalmitis. El principal análisis estadístico ha consistido en un análisis de varianza de medidas repetidas para medir la evolución de la agudeza visual y del cambio medio de la agudeza visual en las sucesivas inyecciones; y una regresión lineal para valorar los factores asociados a la agudeza visual final. Además de este estudio observacional, se presenta la evolución de dos casos clínicos con pronósticos diferentes. De este estudio se puede concluir que las inyecciones con corticoides mantienen la agudeza visual estable y que disminuye el edema macular, sin la presencia de ninguna complicación que obligue a la suspensión del tratamiento. Por tanto, el implante de dexametasona intravítreo es efectivo y localmente seguro para el tratamiento y manejo del edema macular diabético, refractario a fármacos antiangiogénicos en monoterapia o combinado con corticoide intravítreo.
