Abstract

Una correcta orientación etiológica inicial de la meningitis mejora el pronóstico de los pacientes y evita hospitalizaciones y tratamientos innecesarios. Con objeto de identificar con seguridad los pacientes susceptibles de manejo ambulatorio, se diseñó el Bacterial Meningitis Score (BMS). Por otro lado, la procalcitonina (PCT) es un biomarcador útil para diferenciar meningitis vírica y bacteriana. La meningitis viral tiene un curso benigno en la mayoría de las ocasiones, pudiendo ser manejados estos pacientes de forma ambulatoria. De esta forma, si un paciente cumple una serie de requisitos podría ser manejado ambulatoriamente. Aunque en muchas ocasiones los pacientes que cumplen estos criterios son manejados de manera ambulatoria, no existen estudios que validen el manejo ambulatorio de pacientes con criterios de bajo riesgo de meningitis bacteriana. Objetivo principal: Analizar la evolución de pacientes de 2 a 14 años diagnosticados de meningitis con criterios de muy bajo riesgo de meningitis bacteriana en los servicios de urgencia pediátricos y que fueron manejados ambulatoriamente sin antibióticos. CONCLUSIONES: Una combinación de criterios clínicos, BMS y PCT permite el manejo ambulatorio seguro, sin antibióticos de alrededor del 20% de los niños con meningitis. La sustitución en el BMS del número absoluto de neutrófilos por la PCT facilitaría el manejo ambulatorio de otro importante número de pacientes. Son necesarios estudios más amplios que corroboren estos resultados.
