Abstract

CAT- Aquesta tesis presenta un compendi de quatre articles originals realitzats durant els anys 2019-2025. Amb aquests es contribueix a la investigació desenvolupada per abordar la hipòtesi de si la vacunació contra el virus del papil·loma humà (VPH) comporta un augment del risc de desenvolupar certes malalties amb un possible component autoimmune, com la tiroïditis autoimmune i la malaltia inflamatòria intestinal (MII), en nenes i dones adolescents. Es van dur a terme dos estudis de cohort retrospectius utilitzant la Base de dades per a la Investigació Farmacoepidemiològica en l’Àmbit Públic (BIFAP), un per a cada esdeveniment (tiroïditis i MII). La població principal d’estudi va incloure nenes i dones adolescents (de 9 a 18 anys) entre 2007 i 2016. Es va estimar el risc de tiroïditis i MII en els períodes de vacunació i postvacunació en comparació amb els períodes sense vacunació. En aquesta tesi, en general, no s’ha observat un increment del risc de tiroïditis des del moment de la vacunació fins dos anys després en comparació amb períodes sense vacunació. Pel que fa a la MII, només s’ha observat un increment de risc immediatament després de la primera dosi, el qual es pot atribuir a la recomanació clínica de vacunar quan se sospita que la pacient ja pateix MII. Finalment, mitjançant aquests estudis, la tesi aporta evidència sobre aspectes de seguretat de la vacunació contra el VPH, aspectes metodològics en la recerca farmacoepidemiològica i factors determinants en l’administració d’aquesta vacuna en la població objectiu dins del sistema sanitari espanyol

ENG- This thesis presents a compendium of four original articles conducted between 2019- 2025. They contribute to the body of research aimed at addressing the hypothesis of whether vaccination against the human papillomavirus (HPV) leads to an increased risk of developing certain diseases with potential autoimmune component, such as autoimmune thyroiditis and inflammatory bowel disease (IBD), in girls and adolescent women. Two retrospective cohort studies were carried out using the Pharmacoepidemiological Research Database for Public Health System (BIFAP), one for each outcome (thyroiditis and IBD). The main study population included girls and adolescent females (aged 9 to 18) between 2007 and 2016. We estimated the risk of thyroiditis and IBD in the vaccination and post-vaccination periods compared to unvaccinated periods. In this thesis, in general, no increase in the risk of thyroiditis was observed from the time of vaccination up to 2 years afterward compared to unvaccinated periods. Regarding IBD, an increase in risk was observed only following the first dose, which is likely attributable to the clinical recommendation to vaccinate upon onset of IBD symptoms. Finally, through these studies, the thesis contributes evidence on the safety aspects of HPV vaccination, methodological aspects in pharmacoepidemiology research, and determining factors in the administration of this vaccine to the target population within the Spanish healthcare system
