Abordaje multidisciplinar para el diagnóstico y caracterización de enfermedades musculares raras

Abstract

Esta tesis doctoral por compendio incluye tres artículos que representan contribuciones relevantes en el campo de las enfermedades musculares: 1) Descripción de una nueva enfermedad muscular (identificación del gen ATP2A2, implicado en una nueva forma de miopatía proximal relacionada con la homeostasis del calcio), con descripción del fenotipo clínico-patológico y uso de estudios funcionales para demostrar la patogenicidad de la variante identificada. 2) Descripción de dos nuevas familias españolas afectas de una enfermedad muscular descrita recientemente, causada por expansión en el gen PLIN4, de la que existían muy pocos casos descritos previamente. El trabajo permite ampliar el conocimiento del fenotipo asociado a la enfermedad, y describe detalladamente las características miopatológicas que permiten clasificarla dentro del grupo de miopatías con vacuolas autofágicas de características sarcolemales. 3) Ampliación del conocimiento de la fisiopatogenia de la miopatía necrotizante inmunomediada en pacientes en edad pediátrica no expuestos previamente a estatinas. Se analiza por primera vez un grupo de genes relacionados con la biosíntesis del colesterol, sin detectar variantes patogénicas que contribuyan a una sobreexpresión del antígeno HMGCR. Los tres trabajos, publicados en revistas de primer cuartil, aplican un abordaje integral, multidisciplinar y traslacional que combina investigación clínica y básica, con estudio de datos clínicos, radiológicos, histológicos, estudios funcionales para validar nuevas variantes genéticas y técnicas genéticas de última generación para el diagnóstico de nuevas miopatías hereditarias, caracterización de miopatías hereditarias recientemente descritas y ampliación del conocimiento fisiopatológico de la miopatía necrotizante inmunomediada asociada a anti-HMGCR. En resumen, esta tesis doctoral pretende mejorar la caracterización clínica, genética, histológica y radiológica de varias miopatías, así como ampliar el conocimiento fisiopatológico sobre las bases moleculares que las causan.

Aquesta tesi doctoral per compendi inclou tres articles que representen contribucions rellevants en el camp de les malalties musculars: 1) Descripció d’una nova malaltia muscular (identificació del gen ATP2A2, implicat en una nova forma de miopatia proximal relacionada amb la homeostasi del calci), amb descripció del fenotip clínic i patològic, i estudis funcionals per demostrar la patogenicitat de la variant identificada. 2) Descripció de dues noves famílies espanyoles afectes d’una malaltia muscular descrita recentment, causada per expansió al gen PLIN4, de la que existien molt pocs casos descrits prèviament. El treball permet ampliar el coneixement del fenotip associat a la malaltia, i descriure detalladament les característiques miopatològiques que permeten classificar-la dins del grup de miopaties amb vacuoles autofàgiques de característiques sarcolemals. 3) Ampliació del coneixement de la fisiopatogènia de la miopatia necrotitzant immunomediada en pacients en edat pediàtrica no exposats prèviament a estatines. S'analitza per primer cop un grup de gens relacionats amb la biosíntesi del colesterol, sense detectar variants patogèniques que contribueixin a una sobreexpressió de l'antigen HMGCR. Els tres treballs, publicats en revistes de primer quartil, apliquen un abordatge integral, multidisciplinar i translacional que combina investigació clínica i bàsica, amb estudi de dades clíniques, radiològiques, histològiques, estudis funcionals per validar noves variants genètiques i tècniques genètiques d'última generació per al diagnòstic de noves malalties musculars hereditàries, caracterització de miopaties hereditàries descrites recentment, i ampliació del coneixement molecular de la miopatia immunomediada associada a anti-HMGCR. En resum, aquesta tesi doctoral pretén millorar la caracterització clínica, genètica, histològica i radiològica de diverses miopaties, així com ampliar el coneixement fisiopatològic sobre les bases moleculars que les causen.

This doctoral thesis includes three publications that represent significant contributions to the field of muscle diseases: 1) Description of a novel muscle disease (identification of the ATP2A2 gene, implicated in a new form of proximal myopathy due to impaired calcium homeostasis), with a description of the clinicopathological phenotype and the use of functional studies to demonstrate the pathogenicity of the identified variant. 2) Description of two new Spanish families affected by a recently described muscle disease caused by an expansion in the PLIN4 gene. This work expands our understanding of the clinical phenotype associated with the disease, and describes in detail the myopathological features that allow it to be classified within the group of myopathies with autophagic vacuoles of sarcolemmal features (AVSF myopathies). 3) Expansion of the molecular knowledge of immune-mediated necrotizing myopathy in pediatric patients not previously exposed to statins. For the first time, a group of genes related to cholesterol biosynthesis is analyzed, without identification of pathogenic variants that may contribute to an overexpression of the HMGCR antigen. The three studies, published in top peer-reviewed Q1 journals, apply a comprehensive, multidisciplinary, and translational approach that combines clinical and basic research with the analysis of clinical, radiological, and histological data, functional studies to validate new genetic variants, and cutting-edge genetic techniques for the diagnosis of novel hereditary myopathies, the characterization of recently described hereditary myopathies, and the expansion of molecular understanding of anti-HMGCR-associated immune-mediated necrotizing myopathy. In summary, this doctoral thesis aims to improve the clinical, genetic, histological, and radiological characterization of several myopathies, as well as to broaden the pathophysiological understanding of their underlying molecular mechanisms.
