Abstract

[spa] INTRODUCCIÓN: Las enfermedades pulmonares intersticiales (EPI) son un grupo heterogéneo de enfermedades que comparten algunas manifestaciones clínico-radiológico- patológicas similares. Entre las EPI fibróticas no FPI (neumonitis por hipersensibilidad fibrosante (NHf), EPI asociada a enfermedad autoinmune sistémica (EPI-EAS), neumonía intersticial no específica, sarcoidosis o neumoconiosis, existe un porcentaje de pacientes que presenta progresión de la enfermedad a pesar del tratamiento inicialmente establecido, lo que se conoce como fibrosis pulmonar progresiva (FPP). HIPÓTESIS: La FPP es una característica evolutiva de algunas EPI fibróticas no FPI que presentan características específicas y que difieren de las formas no fibróticas. Sin embargo, estas EPI fibróticas no FPI difieren mucho dependiendo de la entidad y otros factores asociados. Existen marcadores que nos permitirán predecir qué subgrupo de pacientes van a presentar FPP y pronosticar la mortalidad asociada. Los índices multidimensionales de determinación pronóstica y de mortalidad pueden ser útiles en FPP al igual que sucede en la FPI. OBJETIVOS: El objetivo primario es determinar cuáles son los biomarcadores pronósticos de progresión y mortalidad en las formas más frecuentes de FPP: EPI asociada a artritis reumatoide (EPI-AR) y NHf.
