Abstract

En la acromegalia los niveles de GH e IGF1 son nuestra principal herramienta para monitorizar el curso de la enfermedad y el éxito terapéutico. Sin embargo, estos no siempre reflejan el curso clínico de la enfermedad o las comorbilidades asociadas. La presente tesis doctoral tuvo como objetivo identificar metabolitos que podrían usarse como biomarcadores para un mejor fenotipado de la enfermedad. Para este propósito, se examinó mediante un estudio metabolómico no dirigido la huella metabólica en el suero de 30 pacientes con acromegalia y 30 controles pareados de la misma edad. Los pacientes con acromegalia presentaron menos aminoácidos de cadena ramificada (BCAAs) en comparación con el grupo control (valina: 4.75 ± 0.87 vs. 5.20 ± 1.06, p <0.05; isoleucina: 2.54 ± 0.41vs. 2.80 ± 0.51 UAs; p <0.05). Los BCAAs también fueron más bajos en pacientes con enfermedad activa en comparación con pacientes con niveles normales de IGF-1 con o sin tratamiento médico. También detectamos una relación inversa entre los BCAAs y el depósito de grasa epicárdica. Los niveles séricos de GH, pero no IGF-1, se correlacionaron inversamente con la valina y la isoleucina. Estos hallazgos indican que los bajos niveles de BCAAs son la principal huella metabólica de la acromegalia y que GH en lugar de IGF-1 podría ser el mediador principal. Además, nuestros resultados sugieren que la evaluación de los BCAAs podría ayudar a identificar enfermedad activa o comorbilidades residuales y monitorizar la respuesta terapéutica.

En l’acromegàlia els nivells de GH i IGF1 són la nostra principal eina per monitoritzar el curs de la malaltia i l’èxit terapèutic. No obstant això, aquests no sempre reflecteixen el curs clínic de la malaltia o les comorbiditats associades. La present tesi doctoral va tenir com a objectiu identificar metabòlits que podrien usar-se com a biomarcadors per a un millor fenotipat de la malaltia. Per a aquest propòsit, es va examinar mitjançant un estudi metabolòmic no dirigit l’empremta metabòlica en el sèrum de 30 pacients amb acromegàlia i 30 controls aparellat de la mateixa edat. Els pacients amb acromegàlia van presentar menys aminoàcids de cadena ramificada (BCAAs) en comparació amb el grup control (valina: 4.75 ± 0.87 vs. 5.20 ± 1.06, p <0.05; isoleucina: 2.54 ± 0.41vs. 2.80 ± 0.51 UAs; p <0.05 ). Els BCAAs també van ser més baixos en pacients amb malaltia activa en comparació amb pacients amb nivells normals d’IGF-1 amb o sense tractament mèdic. També vam detectar una relació inversa entre els BCAAs i el dipòsit de greix epicàrdica. Els nivells sèrics de GH, però no IGF-1, es van correlacionar inversament amb la valina i la isoleucina. Aquestes troballes indiquen que els baixos nivells de BCAAs són la principal empremta metabòlica de l’acromegàlia i que GH en lloc d’IGF-1 podria ser el mediador principal. A més, els nostres resultats suggereixen que l’avaluació dels BCAAs podria ajudar a identificar malaltia activa o comorbiditats residuals i monitoritzar la resposta terapèutica.

IGF1 and GH serum levels are the main targets for monitoring acromegaly activity and the successful of treatment. However, there is not a close relationship between IGF and GH circulating levels and clinical course of the disease and the associated comorbidities. The present doctoral thesis was aimed at identifying metabolites that could be used as biomarkers for a better disease phenotyping. For this purpose, metabolic fingerprint using untargeted metabolomic approach was examined in serum from 30 patients with acromegaly and 30 age-matched controls. Patients with acromegaly presented less branched chain amino-acids (BCAAs) compared to the control group (valine: 4.75 ± 0.87 vs. 5.20 ± 1.06, p<0.05; isoleucine: 2.54 ± 0.41vs. 2.80 ± 0.51 AUs; p<0.05). BCAAs were also lower in patients with active disease compared to patients with normal levels of IGF-1 with or without medical treatment. In addition, an inverse relationship was found between BCAAs and the deposit of epicardic fat assessed by echography. GH, but not IGF-1 serum levels were inversely correlated with both valine and isoleucine. These findings indicate that low levels of BCAAs is the main metabolic fingerprint of acromegaly and that GH rather than IGF-1 might be the primary mediator. In addition, our results suggest that the assessment of BCAAs could help to identify active disease or residual comorbidities and to monitor the response to therapeutic strategies.
