Abstract

El avance en los sistemas sanitarios, en los conocimientos médicos y lasmejoras en la calidad de vida, han contribuido a que la expectativa de vida delas personas en los países desarrollados pueda superar los 80 años. Esto hapermitido observar el aumento de la frecuencia de enfermedades comunes enla tercera edad, de las cuales, una de las más relevantes es la demencia.Las dos etiologías más frecuentes de la demencia son la degenerativa, con laenfermedad de Alzheimer (EA) como causa principal, y las de etiologíavascular o demencia vascular, dentro de la cual se encuentra con unaprevalencia reducida la encefalopatía arterioesclerótica subcortical oenfermedad de Binswanger (EB).Dado que, por una parte, el diagnóstico de las demencias no es suficiente nidefinitivo por medios clínicos, y por otra parte, el diagnóstico patológico nomodifica la evolución de la enfermedad, actualmente se pone énfasis en eldiagnóstico por estudios de neuroimagen. En los últimos años, con el advenimiento de la tomografía computarizada (TC)y la resonancia magnética (RM), se han podido observar lesiones en lasustancia blanca de los hemisferios cerebrales de pacientes de estos dosgrupos etiológicos de demencia, la degenerativa y la vascular, así como en pacientes de edad avanzada sin deterioro cognitivo con o sin factores de riesgovascular.
