Estudio biomecánico de los implantes utilizados en la reparación ístmica de la espondilolistesis

Abstract

Los implantes empleados en la reparación ístmica de la espondilolistesis deben presentar unas propiedades mecánicas adecuadas con el fin de proporcionar una estabilidad inmediata a nivel de la zona lítica. Hemos realizado un estudio biomecánico de elementos vertebrales de cadaver conservados por congelación en los cuales fue reproducido el defecto lítico. Fueron estudiadas tres técnicas de reparación diferentes: el atornillado translaminar, el cable a compresión y la pinza pedículo-laminar. Todos los especímenes fueron ensayados mediante una máquina universal de tracción-compresión, obteniéndose unas curvas fuerza-deformación a partir de las cuales se ha realizado el cálculo de la rigidez de los elementos vertebrales estudiados.El sistema de la pinza pedículo-laminar ha mostrado ser el más rígido proporcionando el 60% de la rigidez de la vértebra humana intacta, mientras que la técnica del atornillado translaminar ha proporcionado el 37%. El sistema del cable a compresión ha proporcionado unas curvas fuerza-deformación cuya rigidez no resulta comparable con los otros dos métodos de reparación.

The devices used for isthmic repair in spondylolisthesis must have adequate mechanical features in order to provide immediate stability in the lytic area.A biomechanical study was performed using frozen preserved human vertebrae in which the lytic defect was reproduced. Three diferent techniques (translaminar screw, cable with compression and pediculo laminar screw-hook) were assayed. All specimens were tested in a universal traction- compression machine obtainig strength-deformation curves that were used to obtain stifness values for the tested vertebrae.The pediculo laminar screw-hook technique provided the stiffest repair achieving 60% of the stifness provided by a human intact vertebra, while the translaminar screw provided 37%. The strength- deformation values obtained by the cable with compression repair technique were not comparable to those obtained by the two other techniques.
