Efecto del plasma rico en plaquetas en la osteogénesis a distracción

Abstract

La Osteogénesis a Distracción es un procedimiento por el cual se forma tejido óseo en longitud mediante la realización de fuerzas distractoras en sentidos opuestos, en el contexto de una fractura u osteotomía. Como todo procedimiento de naturaleza quirúrgica puede presentar ciertas complicaciones, algunas de las cuales están relacionadas con el volumen y madurez del regenerado óseo. Algunas de las investigaciones sobre la Osteogénesis a Distracción han perseguido ahondar en los factores técnicos y biomecánicos que optimizan la formación de hueso. Las investigaciones más recientes han tratado de mejorar la Osteogénesis a Distracción mediante agentes biológicos aplicados local o sistémicamente. El Plasma Rico en Plaquetas es un concentrado de plaquetas de sangre periférica. En la actualidad se está empleando en varios ámbitos de la medicina, ya que su alta concentración en factores de crecimiento podría suponer un beneficio en aquellos procesos terapéuticos en caminados a conseguir una regeneración tisular. Este estudio pretende evaluar si la aplicación de Plasma Rico en Plaquetas en un modelo ovino de Osteogénesis a Distracción estimula la formación de hueso. Para ello se han empleado dos grupos de 10 corderos cada uno. En el grupo control se ha realizado un procedimiento de Osteogénesis a Distracción mediante osteotomía centrodiafisaria del fémur y empleando un fijador externo. El grupo intervención se diferencia del grupo control en que recibió tres dosis de Plasma Rico en Plaquetas autólogo, durante el inicio de la fase de latencia, el inicio de la fase de distracción y a mitad de la fase de distracción. Se evaluó el regenerado óseo mediante radiología simple secuencial, Tomografía Computerizada del callo de elongación (estudio morfométrico del fémur y del callo, estudio de densidad ósea en UH), estudio de histología simple (cuantificación celular de osteoblastos y osteoclastos, cuantificación de neoangiogénesis y determinación de madurez trabecular y del callo) e histomorfometría. No se evidenciaron diferencias entre los dos grupos, no pudiéndose afirmar que el Plasma Rico en Plaquetas estimule la formación de hueso ni la maduración del mismo en nuestro modelo ovino de Osteogénesis a Distracción.

Distraction Osteogenesis is a procedure in which it is formed bone in length by performing a distraction force in opposite directions, in the context of a fracture or osteotomy. As surgical procedure may have somewhat nature complications, some of which are related to the volume and maturity of the regenerated bone. Some of the research on Distraction Osteogenesis have sought to delve into the technique and biomechanic factors to optimize bone formation. The recent research has attempted to improve the Osteogenesis Distraction by biological agents applied locally or systemically. Platelet Rich Plasma is a concentrate of platelets of peripheral blood. It is currently being used in diferent fields of medicine, because the high concentration of growth factors could provide a therapeutic benefit in those processes related with tissue regeneration. This study aims to assess if the implementation of Platelet-Rich Plasma in a sheep model of distraction osteogenesis could stimulate bone formation. It has been used two groups of 10 animals each. It has performed a procedure of Distraction Osteogenesis in the control and intervention group with a shaft osteotomy of the femur using an external fixator. The intervention group received three doses of autologous Platelet Rich Plasma, during the onset of latency phase, the beginning of the distraction phase and half of the distraction phase. Bone regeneration was evaluated by sequential simple radiographs, computed tomography of the callus (morphometric study of the femur and callus, bone density study at HU), study of histology (cell counts of osteoblasts and osteoclasts, quantification and determination neoangiogenesis, trabecular and callus maturity) and histomorphometry. There was no difference between the two groups. It can not be affirmed that the Platelet Rich Plasma stimulates bone formation and the maturation in our sheep model of distraction osteogenesis.
