Salut mental i qualitat de vida en pacients cremats

Abstract

L’objectiu d’aquest estudi és descriure les conseqüències que pot tenir, en la salut mental i enla qualitat de vida, el fet de patir cremades que requereixin de l’hospitalització dels pacients. A més a més, intenta identificar també alguns dels mecanismes que intervenen en l’aparició d’aquest malestar. Per tal de fer-ho, es realitza una revisió de la literatura i es recullen les dades sociodemogràfiques dels participants, les característiques de la lesió i del seu tractament, antecedents mèdics i consum de tòxics al llarg dels sis mesos posteriors al moment de la lesió, mitjançant una entrevista semi-estructurada creada ad-hoc específicament per a aquest projecte. També s’administren diversos instruments d’avaluació per tal de mesurar la personalitat, els símptomes d’ansietat, de depressió, d’estrès traumàtic, l’estat de salut general i la qualitat de vida dels pacients. En aquesta tesi doctoral es confirma la presència de psicopatologia secundària a la cremada i s’assenyala a variables subjectives, com la personalitat o aquelles relacionades amb la creació de memòries traumàtiques, com a factors de risc. Aquests resultats podrien augmentar la comprensió dels efectes psicosocials que tenen les lesions per cremada greu en les seves víctimes i el seu entorn més proper, així com identificar certs factors de vulnerabilitat que podrien ajudar a prevenir les evolucions més tòrpides pel que fa a salut mental i emocional.

The aim of this study is to describe the possible consequences, in terms of mental health and quality of life of patients suffering burns requiring hospitalization. In addition, we attempt to identify some of the mechanisms involved in the appearance of distress among these patients. To do so, a review of the literature was performed and also, demographic data of the participants, nature of the injury and its treatment, medical history and use of toxic substances during the six months after the time of injury, were collected using a semi-structured interview created ad-hoc specifically for this project. In addition, several assessment measurements of personality, anxiety, depression, and posttraumatic stress symptoms, general health status and quality of life were administered. This doctoral thesis confirms the presence of psychopathology secondary to burns and points to subjective variables, such as personality or those related to the creation of traumatic memories, as risk factors. These results may increase understanding of the psychosocial effects due to serious burn injuries on their victims and their relatives, and identify certain variables of vulnerability that could help to prevent more torpid evolution with respect to mental and emotional symptoms.
