La Heterogeneidad de la mortalidad como indicador de la salud comunitaria.

Abstract

[spa] Se desarrolla la metodología de aplicación del índice de diversidad de Shannon-Wiener a estudios de mortalidad. Su ventaja es que resume en una cifra la distribución de las defunciones por grupos de causas. El objetivo de estudio es validar la aplicación del índice de diversidad a la mortalidad para: a) diferenciar distribuciones de mortalidad por grupos de causas de muerte, y b) complementar la tasa de mortalidad como indicador sanitario. La memoria se presenta en cuatro capítulos. Capítulo 1. Analiza la idoneidad de los principales índices de diversidad para la selección de uno: Shannon-Wiener. Estudia el efecto sobre la diversidad de dos sistemas distintos de clasificación de grupos de causas. Valora el significado, condicionantes y efectos de aplicar ó no la estandarización. Capítulo 2. Desarrolla la metodología necesaria para analizar la evolución conjunta de la diversidad y la tasa de mortalidad con los datos de Cataluña (1975-93). Identifica cambios de diversidad debidos a cambios en la distribución y/ó en la riqueza de grupos de causas. Describe e interpreta la evolución de la tasa de mortalidad y de la diversidad en la población general y subpoblaciones por género y edad. Aporta valores de referencia de la diversidad (H', E65 y E) para la población general y subpoblaciones por género y edad. Capítulo 3. Estudia la distribución espacial de la diversidad de la mortalidad por grupos de causas en combinación con la tasa de mortalidad en las provincias y comunidades autónomas de España. Identifica mejor los patrones geográficos y su interpretación. Capítulo 4. Analiza la tasa de mortalidad y la diversidad de la mortalidad por causas MIPSE en la población de Cataluña (1975-95). Determina si los cambios en diversidad aportan ó no información complementaria para su gestión. Detecta las diferencias importantes en diversidad y sus patrones evolutivos. Como resultado global del estudio, proponemos que los Anuarios de Mortalidad incluyan los valores de diversidad. La aplicación conjunta de ambos indicadores mejora la valoración del estado de salud de la población para la gestión de recursos.
