Control percibido y reactividad de la presión intraocular

Abstract

En la presente tesis se realiza una aproximación desde la perspectiva de la Psicofisiología y de la Psicología de la Salud al estudio de la reactividad de la presión intraocular (PIO) al estrés agudo de tipo cognitivo y al estrés psicosocial. La inclusión de un estresor psicosocial en el estudio de la reactividad de la PIO constituye una de las novedades de este trabajo, dada la ausencia de literatura previa en la que empleen este tipo de tareas. Además se evalúa por primera vez el posible papel modulador que ejercen los juicios de evaluación cognitiva, en términos de percepción de control desde la teoría transaccional de Lazarus y Folkman (1986), y la teoría cognitivo social postulada por Albert Bandura. Algunos de los hipotéticos determinantes evaluados en los 3 estudios que componen esta tesis (1 experimento y 2 estudios de campo) son: la competencia percibida, la percepción de amenaza y la percepción de reto). También se evalúa el posible efecto que la ansiedad rasgo y estado podrían desempeñar en la modulación de esta reactividad. Por otra parte se realiza una caracterización preliminar de un fenómeno análogo al conocido como efecto de Hipertensión de bata blanca (HBB) que nosotros denominamos Hipertensión ocular de bata blanca (HOBB), consistente en una habituación de la PIO, que presenta un patrón decreciente cuando se obtienen medidas sucesivas de este parámetro. Además, se realiza una aproximación a algunos de los hipotéticos determinantes cognitivos de este fenómeno como la percepción de control y de amenaza y la ansiedad rasgo y estado. Los resultados de este trabajo sugieren que la PIO es reactiva tanto al estrés cognitivo como psicosocial, siendo el valor del incentivo (en este caso la valencia emocional asociada a un eventual fracaso en la tarea) una variable que podría ejercer un papel modulador de esta respuesta, y que la percepción de control y de amenaza constituyen dos posibles variables explicativas de la HOBB. En la discusión final de esta tesis se comentan las limitaciones de los estudios que la componen, y se reflexiona respecto a la pertinencia de extrapolar este tipo de estudios al ámbito clínico, dada la importancia que estos hallazgos sugieren la necesidad de evaluar el papel del estrés en la evaluación y tratamiento de la hipertensión ocular y del glaucoma.

This thesis is an approach from the perspective of Psychophysiology and Health Psychology to the study of the reactivity of intraocular pressure (IOP) to acute cognitive and psychosocial stress, The inclusion of a psychosocial stressor in the field of the IOP reactivity is one of the novelties of this work, given the lack of previous literature employing these type of tasks. It also evaluates for a first time, the potential the modulating role of cognitive evaluation judgments in terms of perceived control from the transactional theory of Lazarus and Folkman (1986 ), and social cognitive theory postulated by Albert Bandura (1977). Some of the hypothetical determinants evaluated in the 3 studies of this thesis (one experiment and two field studies) are: perceived competence, perceived threat and perceived challenge). We also evaluate the possible effect that state and trait anxiety might play in modulating this reactivity. Moreover, we make a preliminary characterization of a phenomenon analogous to white coat hypertension (WCH), that we call white coat ocular hypertension (WCOH), consisting in a habituation of IOP, which has a decreasing pattern when successive measurements are obtained for this parameter. Furthermore, it is an approach to some of the hypothetical cognitive determinants of this phenomenon as the perception of threat and control and state and trait anxiety. The results of this study suggest that IOP is reactive both cognitive and psychosocial stress. The value incentive value (in this case the emotional valence associated with a potential failure in the experimental task) seems to be a variable that could exert a modulating role on this response, and that the perception of control and threat are two possible explanatory variables for WCOH. In the final discussion of this thesis we expose the limitations of our studies, and we suggest the importance of extrapolating these studies to the clinical setting, given the importance of these findings, that suggest the need to evaluate the role of stress in the evaluation and treatment of ocular hypertension and glaucoma.
