Os carcinomas da tireóide são os mais frequentes do sistema endócrino, e compreendem um grupo heterogéneo de neoplasias com características clínicas, anátomo-patológicas e moleculares distintas. Os mais frequentes são os derivados do epitélio folicular (carcinoma papilar, carcinoma folicular, carcinoma pouco diferenciado e carcinoma anaplásico), sendo mais raros os derivados do epitélio parafolicular (carcinoma medular). No carcinoma papilar, a mutação mais frequente é a mutação do BRAF, enquanto que os rearranjos cromossómicos envolvem os genes RET e NTRK1. Uma elevada prevalência de mutações RAS e de rearranjos do gene PAX8/PPAR são observadas na variante folicular do carcinoma papilar e no carcinoma folicular. Os conhecimentos acumulados na compreensão dos eventos genéticos e moleculares que envolvem os tumores de tireóide ainda não são suficientemente conhecidos, persistindo o desafio no sentido de se obterem marcadores que auxiliem no diagnóstico na avaliação do prognóstico, na tentativa de obter terapêuticas mais eficazes para os doentes com este tipo de neoplasias.
Thyroid carcinomas are the most frequent malignancies of the endocrine system, comprising a heterogeneous group of neoplasms with distinct clinical, pathological and molecular characteristics. The most common thyroid carcinomas are derived from the follicular epithelium (papillary carcinoma, follicular carcinoma, poorly differentiated carcinoma and anaplastic carcinoma). Carcinomas derived from the parafollicular epithelium (medullary carcinoma) are rare. The most frequent mutation in papillary carcinoma is the mutation of BRAF gene, while chromosomal rearrangements involve RET and NTRK1 genes. A high prevalence of RAS mutations and PAX8/PPAR gene rearrangements is observed in the follicular variant of papillary carcinoma and in follicular carcinoma. Despite the great amount of information on the genetic and molecular events of thyroid tumours, it still remains the challenge to obtain molecular markers that could be useful in diagnosis and in prognostic evaluation, in order to select a better target therapy of the patients.
