Litos Retenidos Posterior a Colecistectomía Laparoscópica y sus Complicaciones

Abstract

La colelitiasis es una de las patologías más frecuentes a nivel mundial y se estima que afecta aproximadamente entre un 10% a 20% de la población. En la actualidad el manejo de elección para el tratamiento de la colelitiasis sintomática es la colecistectomía laparoscópica. Conforme la tendencia mundial incrementa en favor del desarrollo de las técnicas quirúrgicas mínimamente invasivas, también lo hacen las complicaciones asociadas a estas. Las complicaciones más frecuentes asociadas a la colecistectomía laparoscópica son las lesiones de vía biliar y la perforación transoperatoria de la vesícula biliar. Esta última se asocia con el derrame de litos a la cavidad abdominal, los cuales en muchos de los casos no son extraídos en su totalidad y, como consecuencia, quedan retenidos en diversos sitios intraperitoneales. En las últimas décadas se han reportado múltiples casos de complicaciones asociados a estos litos retenidos que van desde procesos de infecciones intraabdominales o torácicas, hasta obstrucción intestinal o trombosis de vasos sanguíneos. Debido a la amplia variedad patológica con la que se presentan las complicaciones asociadas a litos retenidos, no existe en la actualidad información clara acerca de la incidencia con que se presentan y el manejo apropiado para el tratamiento de dichas complicaciones.

Cholelithiasis is one of the most frequent pathologies worldwide and affects approximately 10% to 20% of the population. Currently, the gold standard for treatment of symptomatic cholelithiasis is laparoscopic cholecystectomy. As the global tendency increases in favor of the development of minimally invasive surgical techniques, so does the associated complications. The most frequent complications associated with laparoscopic cholecystectomy are bile duct injuries and gallbladder perforation during surgery. The latter is associated with the spillage of stones into the abdominal cavity, which in many cases are not fully extracted, remaining retained in various intraperitoneal sites. In recent decades, multiple cases of complications associated with these retain stones have been reported, ranging from intra-abdominal or thoracic infections to intestinal obstruction or thrombosis of blood vessels. Due to the wide pathological variety with which complications associated with retaining stones present, there is currently no clear information about the incidence with which they occur and the appropriate management for treating these complications.
