Abstract

Resumen La cirugía de reconstrucción en ortopedia y la ortoplástica, que incluye el manejo de deformidades adquiridas, congénitas, secundarias a trauma, no uniones y procesos infecciosos, ha ido avanzando en las últimas dos décadas, dando énfasis en las fracturas expuestas y los defectos postraumáticos asociados a lesiones en tejidos blandos, lo que sigue considerándose un gran problema terapéutico. Además, cuenta con tasas descritas de pérdidas óseas de hasta 11,4 %, donde la tibia es el hueso afectado hasta en dos tercios de los casos. Así mismo, en la historia, la amputación en estos casos era el tratamiento que se ofrecía, debido a las lesiones tan extensas que llegaban a presentar. Por el contrario, actualmente, se ofrecen diferentes procedimientos de reconstrucción o salvamento, que están basados en el método de osteogénesis por distracción descrito por primera vez por Ilizarov y, posteriormente, popularizado por cirujanos del oeste; el cual ha llegado a revolucionar el manejo de estas lesiones. Pero que representan procedimientos quirúrgicos, con periodos de tratamiento largos y estresantes para el paciente, con altas tasas de no uniones y de infecciones. A partir de lo anterior, este estudio se propone ahondar en los conocimientos respecto al manejo quirúrgico de fracturas expuestas, con compromiso severo de tejido blando, desde una perspectiva de reconstrucción y salvamento, con la finalidad de brindar como opción quirúrgica, el mantener una deformidad ósea o provocarla, para mejorar el estado de los tejidos de cobertura de la zona afectada.

Abstract Reconstructive surgery in orthopedics and orthoplastic, which includes the management of acquired and congenital deformities, secondary to trauma, nonunions, and infectious processes, has progressed in the last two decades. Giving emphasis on open fractures and post-traumatic defects associated with soft tissue injuries continues to be considered a major therapeutic problem. With described rates of bone loss of up to 11,4%, and where the tibia is the affected bone in up to two thirds of cases. In history, amputation in these cases was the treatment that was offered, due to the extensive lesions that these lesions would present. Currently, different reconstruction or salvage methods are offered, which are based on the distraction osteogenesis method first described by Ilizarov, and later popularized by western surgeons, and which has come to revolutionize the management of these injuries. But they represent surgical procedures, with long and stressful treatment periods for the patient, with high rates of non-unions and infections.
