Abstract

Existen muchos riesgos inherentes con la cirugía de reemplazo articular, uno de estos es la pérdida sanguínea. Debido a las complicaciones potenciales y efectos adversos asociados con la transfusión de sangre es imperativo explorar estrategias para el manejo y conservación del sangrado. Los antifibrinolíticos se han usado de manera exitosa para reducir los requerimientos de transfusión en los pacientes de artroplastia electiva. El ácido tranexámico y el ácido épsilon aminocaproico son dos agentes que han demostrado reducir el sangrado perioperatorio y la tasa de transfusión. Existe heterogeneidad de resultados cuando se compara el ácido tranexámico y el ácido épsilon aminocaproico. Lo que sí es consistente es que no hay una diferencia significativa en los requerimientos de transfusión cuando se comparan ambos antifibrinolíticos, así como tampoco hay una diferencia significativa en la incidencia de efectos adversos.
