Artrodiastasis de tobillo como opcion de tratamiento en artrosis postraumatica: indicaciones, tecnica quirurgica y resultados

Abstract

La osteoartrosis traumática de tobillo es una condición de destrucción articular que afecta gran cantidad de pacientes, principalmente personas jóvenes, ya que comúnmente se asocia a traumatismos de alta energía o inestabilidad recurrente de tobillo. Muchos tratamientos han sido descritos en la literatura y se dividen en: 1. Procedimientos que conservan la articulación (artroscopía de tobillo, artrodiastasis, resurfacing osteocondral, osteotomías correctoras) 2. Procedimientos que no conservan la articulación (remplazo de tobillo, artrodesis). La artrodesis de tobillo y el remplazo articular son los tratamientos tradicionales en pacientes con osteoartrosis avanzada y se realizan con el objetivo de aliviar el dolor, sin embargo, a largo plazo pueden acarrear complicaciones en pacientes jóvenes. Para pacientes jóvenes y seleccionados con osteoartrosis postraumática, la artrodiastasis de tobillo puede ser una opción de tratamiento, con buenos resultados a corto y mediano plazo. Para esto, se utiliza la fijación externa para realizar tracción a través de la articulación del tobillo, con el objetivo teórico de mejorar las condiciones de regeneración del cartílago articular. Para el estudio se realizó una búsqueda en base de datos que incluyeran las palabras clave: osteoartrosis de tobillo, artrodiastasis de tobillo, distracción articular y líquido articular de tobillo. Se recolectó la literatura descrita hasta diciembre de 2019.
