Abstract

El sangrado uterino anormal y la miomatosis uterina son de las principales causas de consulta por patología ginecológica en el mundo. Corresponden a entidades con alta prevalencia y un costo importante para el sistema de salud. Se dice que la miomatosis uterina corresponde a la principal causa de tumores pélvicos en mujeres en edad reproductiva, estando presente en al menos del 20–40 % de estas. A lo largo de los años se ha intentado cambiar el enfoque del tratamiento a un manejo más conservador para disminuir el riesgo de un procedimiento quirúrgico y los costos de la atención médica. El sistema intrauterino liberador de levonorgestrel (SIU - LNG) ha demostrado ser eficaz como alternativa para el manejo de esta patología. En la ausencia de patología estructural del útero, el SIU - LNG reduce la pérdida de sangrado menstrual de manera significativa. Con el presente estudio se pretende evaluar de manera objetiva la eficacia y satisfacción del uso de SIU – LNG en las pacientes del servicio de ginecología del Hospital México del año 2022 - 2023, con base en parámetros objetivos como lo son los niveles de hemoglobina. Mediante la revisión de 165 expedientes y del análisis estadístico de los datos obtenidos se logró demostrar, en esta población, la eficacia del uso de del SIU - LNG tanto de manera objetiva, como de manera subjetiva. Se realizó una comparación entre los valores de hemoglobina previos y posteriores a la colocación del dispositivo donde se demostró un incremento en este valor, tanto en la población con miomatosis como en la población sin este diagnóstico. Además, también se comprobó la satisfacción de las usuarias con su uso desde el punto de vista de disminución en el sangrado, donde la mayoría reportó una disminución en este.

Abnormal uterine bleeding and uterine myomatosis are among the main causes of gynecological consultations worldwide. They are highly prevalent conditions and represent a significant cost to the healthcare system. Uterine myomatosis is said to be the main cause of pelvic tumors in women of reproductive age, being present in at least 20–40% of them. Over the years, efforts have been made to shift the treatment approach to a more conservative management to reduce the risk of a surgical procedure and healthcare costs. The Levonorgestrel Intrauterine System (LNG-IUS) has proven to be effective as an alternative for the management of this pathology. In the absence of structural uterine pathology, the LNG-IUS significantly reduces menstrual blood loss. This study aims to objectively evaluate the efficacy and satisfaction of the use of the LNG-IUS among gynecology patients at Hospital México in 2022-2023, based on objective parameters such as hemoglobin levels. Through the review of 165 medical records and statistical analysis of the data obtained, the efficacy of the levonorgestrel-releasing device was demonstrated in this population, both objectively and subjectively from the user's perception. A comparison was made between hemoglobin levels before and after the placement of the device, showing an increase in this value, both in the population with myomatosis and in the population without this diagnosis. Additionally, user satisfaction with its use was also confirmed from the perspective of reduced bleeding, with the majority reporting a decrease in this symptom.
