Abstract

La enfermedad celíaca (EC) es una enfermedad crónica y sistémica cuya incidencia está aumentando en los últimos años, situándose la prevalencia mundial de la enfermedad en torno al 1-3 %. Actualmente, el único tratamiento es un cambio en el estilo de vida mediante la instauración de una dieta sin gluten (DSG) estricta que, normalmente, en pocos días o semanas produce una mejoría clínica que precede a la normalización serológica y también a la desaparición de la atrofia vellositaria característica de la enfermedad. Actualmente hay en marcha multitud de estudios sobre el enfoque terapéutico de la enfermedad, tanto a nivel de la “cascada inmunológica” como a nivel de la barrera intestinal, los triggers ambientales o la inducción de tolerancia al gluten. No obstante, el hecho de que la DSG sea el único tratamiento disponible hace indispensable su correcta realización y un adecuado seguimiento, puesto que, aunque no es en términos generales de fácil cumplimiento, la falta de adherencia a la DSG puede dar lugar a la aparición de una enteropatía asociada al linfoma de células T, entre otras cosas.
