Abstract

INTRODUCCIÓN: La onicocriptosis es una de las onicopatías de mayor prevalencia en consulta de podología. Los tratamientos conservadores son la primera elección terapéutica, surgiendo remodelación ungueal con gel de clotrimazol y las infiltraciones de glucocorticoide perilesional como una alternativa, aunque se desconoce su eficacia a medio plazo. Así pues, el objetivo de este trabajo fue comparar de forma aislada, la eficacia de ambas técnicas comparadas con el mechado de venda de gasa para el tratamiento conservador de la onicocriptosis. METODOLOGÍA: Se plantean dos estudios analíticos tipo ensayo clínico aleatorio. Por una parte, para la remodelación ungueal se seleccionó una muestra de 20 sujetos que presentaban onicocriptosis en estadio I y IIA y se distribuyeron aleatoriamente en dos grupos, siendo evaluada la presencia de recidiva en un período de 3 meses. Por otro lado, la muestra se compuso de 40 pacientes con onicocriptosis en estadio I, II y III; fueron asignados aleatoriamente a uno de los dos grupos de tratamiento, revisando la reducción del dolor e inflamación a la semana y al mes. RESULTADOS: Tras el periodo de seguimiento de 3 meses, 7 pacientes del grupo reeducación presentaron recidiva de la onicocriptosis, mientras que del grupo remodelación sólo 1 paciente. Sin embargo, Pasado un mes del tratamiento, el grupo corticoide presentó una reducción de la inflamación fue significativamente menor en el grupo corticoide (p= 0.029). CONCLUSIÓN: La técnica de remodelación ungueal presenta una tasa de recidiva inferior a la técnica de reeducación ungueal con venda de gasa. La infiltración de corticoide mostró ser más efectiva que la aplicación de mecha de gasa en la reducción de la inflamación en pacientes con onicocriptosis, aunque parece que una sola infiltración no es suficiente para evitar la recidiva de la patología.

INTRODUCTION: Onychocryptosis is one of the most prevalent onychopathies in podiatry consultations. Conservative treatments are the first therapeutic choice, with nail remodeling with clotrimazole gel and perilesional glucocorticoid infiltrations emerging as an alternative, although their medium-term effectiveness is unknown. Therefore, the objective of this work was to compare, in isolation, the effectiveness of both techniques compared to gauze bandage for the conservative treatment of onychocryptosis. METHODOLOGY: Two randomized clinical trial type analytical studies are proposed. On the one hand, for nail remodeling, a sample of 20 subjects who presented onychocryptosis in stages I and IIA were selected and randomly distributed into two groups, with the presence of recurrence being evaluated over a period of 3 months. On the other hand, the sample was made up of 40 patients with onychocryptosis in stages I, II and III; They were randomly assigned to one of the two treatment groups, reviewing the reduction in pain and inflammation at one week and one month. RESULTS: After the 3-month follow-up period, 7 patients in the reeducation group presented a recurrence of onychocryptosis, while only 1 patient in the remodeling group. However, after one month of treatment, the corticosteroid group presented a reduction in inflammation that was significantly less in the corticosteroid group (p= 0.029). CONCLUSION: The nail remodeling technique has a lower recurrence rate than the nail re-education technique with gauze bandage. Corticosteroid infiltration was shown to be more effective than the application of a gauze wick in reducing inflammation in patients with onychocryptosis, although it seems that a single infiltration is not enough to prevent recurrence of the pathology.
