Abstract

Introducción: Por su confiabilidad y pocos efectos adversos, la anestesia regional subaracnoidea se ha convertido en la técnica de elección para cesárea. Múltiples variables alteran el nivel de bloqueo sensitivo obtenido y la aparición de hipotensión como principal efecto secundario; algunos autores consideran que la velocidad de infusión altera estos parámetros, pero no se ha logrado demostrar en la práctica. Objetivo: Determinar la velocidad de inyección de anestésico local subaracnoideo apropiada durante una cesárea para evitar hipotensión y eventos adversos. Metodología: se diseñó un estudio de cohortes, prospectivo, incluyendo 107 gestantes a término programadas para cesárea no emergente y uso de bloqueo subaracnoideo en condiciones similares, dividiéndolas en un grupo de infusión rápida (25 a 35 segundos) y uno de infusión lenta (55 a 65 segundos); midiendo presión arterial media, nivel anestésico alcanzado, nauseas y vómito. Resultados: Entre las variables de edad, composición corporal, semanas de gestación, paridad, ayuno y tiempo en trabajo de parto se encontró homogeneidad entre ambos grupos. No hubo diferencias en la aparición de náuseas (P 0,36) o vómito (P 0,117). El grupo de infusión lenta presentó menos hipotensión a los 5 minutos (P 0,027); sin diferencia a los 10 (P 0,36) y a los 15 minutos (P 0,063). El nivel sensitivo alcanzado fue más bajo en el grupo de infusión lenta (P 0,014). Conclusiones: El efecto protector de la infusión lenta de anestésico local frente a la hipotensión es temporal y no disminuye la aparición de efectos adversos con respecto a la inyección rápida.

Introduction: because of its reliability and few adverse effects, the regional spinal anesthesia has become the technique of choice for cesarean section. Multiple variables alter the level of sensory blockade obtained and hypotension as main side effect; some authors believe that the rate of infusion alter these parameters, but it has not been demonstrated in the practice. Aims: To determine the appropriate rate of injection of local anesthetic during a Caesarean subarachnoid to avoid hypotension and adverse events. Methods: It was designed a cohort study, prospective, including 107 pregnant women at term scheduled for non-emergency cesarean and using subarachnoid block under similar conditions, dividing them in a rapid infusion group (25-35 seconds) and a slow infusion group (55 - 65 seconds); measuring average blood pressure, anesthetic level reached, nausea and vomiting. Results: Among the variables of age, body composition, weeks of gestation, parity, fasting and time in labor, it was found homogeneity between the two groups. There was no difference in the occurrence of nausea (P 0.36), or vomiting (P 0.117). The slow infusion group showed less hypotension after 5 minutes (P 0.027); no difference at 10 (P 0.36) and 15 minutes (P 0.063). The sensitive level achieved was lower in the slow infusion group (P 0.014). Conclusions: The protective effect of the slow infusion of local anesthetic against hypotension is temporary and does not reduce the occurrence of adverse effects regarding rapid injection.
