Abstract

Las infecciones periprotésicas (IPP) constituyen una complicación devastadora en los procedimientos de reemplazo articular de cadera, con un aumento importante de las comorbilidades y de la mortalidad de nuestros pacientes, adicionalmente los procedimientos de revisión, generan riesgos superiores de IPP. Medidas preventivas y de tratamiento han venido siendo utilizadas recientemente para disminuir los riesgos de infección, y como manejo de esta complicación; entre estas la utilización de vehículos reabsorbibles para liberación local de antibiótico, coadyuvando el tratamiento sistémico antimicrobiano. Dentro de este tipo de sustitutos óseos encontramos el sulfato de calcio, con bien conocidas propiedades biológicas de osteoconducción, siendo el STIMULAN® (Biocomposites, Keele, UK) un sulfato de calcio sintético hemihidratado utilizado en el ámbito del tratamiento de defectos óseos, osteomielitis y procedimientos de artroplastia. Por su capacidad Osteoconductiva, al ser completamente reabsorbido y sustituido por hueso nuevo en lechos esqueléticos, además de sus posibles reacciones transitorias de citotoxicidad, nos planteamos la interrogante de su efecto secundario en osificaciones heterotópicas periarticulares. Este estudio describe los resultados radiográficos orientados a clasificar las osificaciones heterotópicas en pacientes sometidos a procedimiento de revisión de reemplazo total de cadera con utilización de STIMULAN® impregnado con antibiótico durante el cierre. Periprosthetic infections (IPP) are a devastating complication in hip joint replacement procedures, with a significant increase of comorbidities and mortality in our patients. In addition, revision procedures generate higher risks of IPP. Preventive and treatment measures have recently come to be used to reduce the risk of infection, and how to manage this complication; among these the use of resorbable vehicles for local release of antibiotic, contributing to the antimicrobial systemic treatment. Within this type of bone substitutes are calcium sulfate, with well-known biological properties of osteoconduction, being STIMULAN® (Biocomposites, Keele, UK) a synthetic calcium sulfate hemihydrate used in the field of treatment of bone defects, osteomyelitis and arthroplasty procedures. Due to its osteoconductive capacity, it is completely resorbed and replaced by new bone in skeletal beds, and along with its possible transient reaction of cytotoxicity, we raised the question of its secondary effect on periarticular heterotopic ossification. This study describes the radiographic results oriented to classify heterotopic ossification in patients undergoing revision procedure of total hip replacement with STIMULAN® impregnated with antibiotic during closing.
