Las autolesiones no suicidas como síntoma transdiagnóstico

Abstract

La evaluación funcional de las autolesiones no suicidas (NSSI) es un asunto de interés en psicología clínica por su elevada prevalencia en adolescentes, su presentación conjunta con otros problemas psicológicos y su edad de inicio cada vez más temprana. El objetivo del presente estudio fue analizar la frecuencia y funciones de las NSSI, así como su relación con la inflexibilidad psicológica y la urgencia negativa. Participaron 294 jóvenes (74,1% mujeres) con media de edad de 20,4 ± 2,11 años. Se evaluaron NSSI, Impulsividad, Ansiedad/Depresión e Inflexibilidad Psicológica (IP). Los datos se analizaron mediante estadística descriptiva, correlacional e inferencial. Los resultados mostraron que el 44% de los participantes se habían autolesionado alguna vez, casi siempre a solas y experimentando dolor físico al realizar la conducta. Las funciones intrapersonales de las NSSI fueron las más referidas (regulación emocional, autocastigo, expresión del sufrimiento emocional y evitación de fenómenos disociativos). Se encontraron correlaciones positivas entre NSSI y trastornos alimentarios, disforia de género, ansiedad/depresión y, especialmente, inflexibilidad psicológica. El análisis de regresión logística binomial informó de que la IP aportaba valor predictivo significativo sobre la presencia de autolesiones. Se discute la necesidad de entender estas conductas dentro de un modelo explicativo transdiagnóstico.
