La transferencia embrionaria diferida para evitar el riesgo de síndrome de hiperestimulación ovárica

Abstract

El síndrome de hiperestimulación ovárica (SHO) es la complicación más frecuente ocasionada por los tratamientos de estimulación ovárica controlada llevados a cabo durante las técnicas de reproducción humana asistida. El SHO consiste en una respuesta ovárica superior a la fisiológica, lo que provoca una condición clínica con una fisiopatología específica. Dicha entidad se caracteriza por un aumento de la permeabilidad capilar, debido a una serie de sustancias vasoactivas liberadas por los ovarios hiperestimulados, que conduce al cambio del fluido del espacio intravascular al tercer espacio. Aunque en la mayoría de los casos es una enfermedad autolimitada, se asocia con una morbilidad significativa y con la muerte materna. Por ello, se propone como medida preventiva del SHO la realización de la transferencia embrionaria diferida sin comprometer el éxito de la fecundación in vitro. De esta manera, al analizar los resultados de las tasas de implantación, las tasas de embarazo y las tasas de aborto, se ha encontrado que la criopreservación embrionaria con la posterior transferencia embrionaria diferida resulta ser una buena opción en las pacientes con riesgo de desarrollar SHO ya que no compromete el éxito de la fecundación in vitro y evita la evolución del SHO.
