Terapia génica en la distrofia muscular de Duchenne [Escarda González]

Abstract

La Distrofia Muscular de Duchenne (DMD) es uno de los trastornos neuromusculares más comunes dentro de la población, se debe a una mutación en el gen de la distrofina situado en el cromosoma X, afectando a la producción de distrofina, lo que degenera los músculos progresivamente y acorta la vida de quienes lo padecen dándoles una esperanza de vida de 30 años. Actualmente, se está tratando con fármacos derivados de glucocorticoides. Sin embargo, a día de hoy, no existe una cura o tratamiento eficaz que mejorar la calidad de vida de las personas que sufren de esta patología. En los últimos años se han desarrollado avances en el campo de la genética que proponen posibles tratamientos más eficaces a los preexistentes, entre ellos destaca la omisión de exón, para lo cual ya se ha desarrollado el medicamento “eteplirsen”, o la lectura forzada de codones de stop con los tratamientos a partir de “gentamicina” o “ataluren”. También, se está considerando la introducción de “mini-distrofinas” y “micro-distrofinas” de forma directa en el cuerpo de los pacientes a través de vectores víricos adeno-asociados (AAV). Además de otros tratamientos basados en nucleasas (ZFN, TALEN o CRISPR) para la reparación celular a través de las vías de unión de extremos no homólogos (NHEJ) o recombinación homóloga (HR).
