Abstract

Los cambios demográficos en los últimos años han generado un cambio en los patrones epidemiológicos, siendo la pluripatología predominante en la actualidad. Eso lleva a un aumento de la dependencia, lo que significa dificultades en el entorno diario. Las personas que necesitan una atención socio-sanitaria y sus familiares esperan recibir una respuesta simultánea, coordinada y estable que cubra sus necesidades. Los trabajadores sociales son clave en la valoración integral, donde la coordinación con los servicios sociales de la comunidad es fundamental para garantizar la continuidad de la atención al alta hospitalaria, teniendo en cuenta el proyecto de vida de cada persona. Este estudio pretende determinar, utilizando Osakidetza como referencia y más específicamente el Hospital Universitario de Donostia, la importancia de la figura del trabajador social sanitario en el equipo interdisciplinar del hospital para brindar una atención centrada en la persona que garantice la continuidad de la atención cuando regrese a la comunidad.

Els canvis demogràfics en els últims anys han generat un canvi en els patrons epidemiològics, sent la pluripatología predominant en l'actualitat. Això porta a un augment de la dependència, la qual cosa significa dificultats en l'entorn diari. Les persones que necessiten una atenció soci-sanitària i els seus familiars esperen rebre una resposta simultània, coordinada i estable que cobreixi les seves necessitats. Els treballadors socials són clau en la valoració integral, on la coordinació amb els serveis socials de la comunitat és fonamental per a garantir la continuïtat de l'atenció a l'alta hospitalària, tenint en compte el projecte de vida de cada persona. Aquest estudi pretén determinar, utilitzant Osakidetza com a referència i més específicament l'Hospital Universitari de Donostia, la importància de la figura del treballador social sanitari en l'equip interdisciplinari de l'hospital per a brindar una atenció centrada en la persona que garanteixi la continuïtat de l'atenció quan torni a la comunitat.

The demographic changes in recent years have generated a change in epidemic patterns, with the prevailing pluripathology currently. That leads to an increase in dependency, which means difficulties in the daily environment. People who need social care and their families expect to receive a simultaneous, coordinated and stable response that meets their needs. The social workers are key in the integral valuation, where the coordination with the social services of the community is fundamental to guarantee the continuity of the attention to the hospital discharge, taking into account the project of life of each person. This study aims to determine, using Osakidetza as a reference and more specifically the University Hospital of Donostia, the importance of the figure of the social health worker in the hospital's interdisciplinary team to provide a person-centered care that guarantees continuity of care when it returns to the community.
