Abstract

El objetivo principal de este proyecto consiste en valorar la evolución de la marcha en niños en edad preadolescente tratados mediante el método de Ponseti frente a los tratados mediante liberación posterior a partir de técnicas de valoración de la marcha mediante análisis biomecánico. Material y Métodos Estudio retrospectivo de casos y controles aprobado por el comité de ética. Grupo 1: 28 niños (39 pies) tratados mediante liberación posterior. Grupo 2: 18 pacientes (31 pies) tratados mediante el método de Ponseti. Grupo 3: 25 pacientes (50 pies) sin ninguna patología muscular o neurológica. Se realizó un análisis de la marcha con el sistema Kinescan/IBV. Las gráficas cinemáticas fueron analizadas de forma visual individualmente. Resultados. Se encontraron diferencias estadísticamente significativas en todos los parámetros espacio-temporales al comparar los grupos patológicos con el grupo control. El análisis demostró que los grupos patológicos presentaban una marcha más lenta, una mayor anchura de paso, menor longitud de paso y mayor tiempo de paso, p =.000. Tras el análisis individual de las curvas cinemáticas, se obtuvo que únicamente 3 pacientes (9%) del grupo de liberación posterior frente a 10 pacientes (36%) del grupo Ponseti presentaron una marcha normal, p=.012. Conclusión.Los pacientes del grupo Ponseti presentan un patrón de la marcha más parecido al grupo control que el grupo de liberación posterior con la ventaja de evitar un gesto quirúrgico.
