Abstract

Una de las causas más frecuente de consulta por dolor de rodilla suele localizarse en el compartimiento anterior, particulannente expuesto a traumas directos y sobrecargas y que describimos como Síndrome de Dolor Anterior. Existe controversia en el tratamiento y no hay una correlación de la clínica con los hallazgos objetivos cuando existen, como podrían ser displasia, inestabilidad patelar, lesión osteocondral, et. Lo:; autores revisan los conceptos biomecánicos, metabólicos, degenerativos y del dolor de un protocolo racional de tratamiento incruento durante tres a seis meses con Crioterapia, Salicilatos, Glicosaminoglicanos por vía parenteral, potenciación muscular en la gama indolora y consejos ergonómicos a largo plazo. Se reitera la validez de la pauta conservadora dada su efectividad en un gran número de casos: En 2/3 desaparecen los síntomas, 1/5 mejoran por dos temporadas (en relación a limitaciones en el trabajo o deporte causal), y 1/5 sin efecto. Estos últimos, las recidivas y la aparición de signos objetivos, son candidatos a la cirugía, que queda limitada a un 15-20% de los casos.

There is controvcrsy about treatmcnt and there are not correlation bctwccn clinical sintoms and objetive lindings when therc are, as dysplasia, patelar instability, ostcochondral lesions, cte. the authors revicwed biomechanical, rnetabolic, clcgcncrativc and dcgcncraliveand pain conccp1ts at a rational protocol of conservative rnanagment cluring 3 to 6 rnonths with, cryothcrapy, salicylatcs, glycosarninoglycans by parcntcral way, muscular potentiation and physic~1I therapy. In 2/3 ofpatients, thc syntoms disappcared, l/S improved and in 1/S without effect. In the last grnup, relapses and ohjctive tinclings, were inclications for surgcry (15·20% of cases).
