Osificaciones heterotópicas tras artroplastia total primaria de rodilla

Abstract

Introducción. La osificación heterotópica tras artroplastia total de rodilla primaria es una complicación poco estudiada. Nos propusimos estudiar su incidencia en la serie de prótesis totales de rodilla de nuestro hospital, y su posible repercusión clínica. Material y método. Valoramos retrospectivamente 798 prótesis totales de rodilla primaria intervenidas entre 1995 y 2003 encontrando 176 casos (22.05%) de osificación heterotópica. Se realizó la clasificación de las osificaciones de acuerdo a cinco propuestas publicadas siendo un número importante de casos de difícil inclusión. Resultados. Solo 17 casos mostraron algún tipo de repercusión clínica a medio plazo; y de ellos, sólo 2 requirieron ante la persistencia de limitaciones funcionales su exéresis quirúgica al año tras su maduración, con un buen resultado tras la misma. Por otro lado, la pertinencia a un grupo determinado de las diferentes clasificaciones no suponía correlación con la relevancia clínica. Conclusiones. En nuetsra serie las osificaciones heterotópicas tras artroplastia total de rodilla primaria son relativamente frecuentes; pero la mayor parte de ellas no muestran repercusión en el resultado final de la cirugía; y aún entre aquellas con cierta repercusión a medio plazo, son muy pocas las que requieren su exéresis quirúrgica y cuando se realiza se obtiene un buen resultado clínico.

Introduction. Heterotopic ossification following pri- mary total knee arthroplasty is a poorly studied complication. It was our purpose to study its incidence in our hospital ́s total knee arthroplasties series, and its clinical repercusion. Materials and method. We retrospectively reviewed 798 pri- mary total knee arthroplasties performed between 1.995 and 2.003, 176 heterotopic ossifications (22,05%) being found. We tried to classify the ossifications according to five reported radiographic classification systems, although it was hard to do in an important number of cases. Results. Only seventeen cases had some clinical repercusion, and barely two of them with persistent functional problems needed surgical excision after a one-year maduration, with a good clinical outcome after it. No correlation was found between the different groups in the different classifications and the clinical rele- vance. Conclusion. In this series we found that heterotopic ossifications after primary total knee arthroplasty are relati- vely frequent, most of them without symptomatic repercus- sion. The few cases that needed surgical excision had good outcome.
