Artroplastia total de rodilla: clásica o mínimamente invasiva

Abstract

La cirugía mínimamente invasiva (MIS) ha penetrado en la opinión de algunos cirujanos de artroplastia total de rodilla (ATR). Su objetivo puede ser la recuperación más rápida de la función del paciente al realizar un tratamiento menos agresivo de las partes blandas de la rodilla. ¿Pero ésto se ha conseguido?. En este trabajo se comparan los resultados de ATR clásica con la MIS a través del desafío de esta última, exponiendo los pros y los contras de está última, insistiendo en la dificultad de orientar correctamente los cortes óseos y la posición de los componentes a través de una "ventana móvil" y de un "baile simbiótico". Otra alternativa de la MIS es la aplicación de técnicasd e navegación que aún no han permitido precisión en los objetivos básicos. Faltan resultados a largo plazo y, por tanto, es importante no dejarse "calentar" por las compañías ni colegas más avanzados sin una base sólida.

The MIS has penetrated in the TKA’field same in ot- her areas of our speciality. The aim searchs the faster func- tional patient recovery, practising a lower aggressive treat- ment of the soft tissue around the knee. But, this objective is really obtained today? The author compare results between classic and MIS TKAs through the challenge of the latter one, insisting on the difficulty to give the adequate bone cuts and the correct position to the components across a “mobile win- dow” and a “symbiotic dance”. Another option is the applica- tion of navigators, but at the present time this option does not permit accuracy. Medium and long-term results are lacking and, therefore, is not good to warm up surgeons by compa- nies and more advanced colleagues with this emerging tech- nique without a solid security
