Abstract

Gracias a los adelantos en el manejo del politraumatizado, la reconstrucción vascular, injertos nerviosos y técnicas de revascularización de las partes blandas, ha aumentado la posibilidad de salvar un "miembro en riesgo". A pesar de ello muchas veces salvar el miembro supone un coste importante, y una morbilidad para el paciente que incluso a veces es letal. Se trata de un tema muy importante por las implicaciones legales que envuelven la decisión de una amputación precoz de una extremidad traumatizada. Aunque más fácil de objetivar, la lesión ósea no determina el factor más decisivo para determinar la viabilidad y función del miembro afecto. Son las lesiones vasculo-nerviosas y especialmente las partes blandas las que juegan un papel decisivo en la predicción de la supervivencia y funcionalidad del miembro. Presentamos 4 casos tratados en nuestro hospital. Analizamos las distintas pautas diagnóstico-terapéuticas realizadas, y de manera retrospectiva calculamos mediante la escala MESS la puntuación de severidad de la extremidad lesionada. Nos parece necesario unificar criterios en cuanto a las escalas de puntuación, de gran utilidad en el difícil momento de valorar la salvación de un miembro severamente traumatizado.

Recent advances in trauma management, vascular reconstruction, nerve grafting and vascular tissue transfer have dramatically extended the possibility to preserve a mangled limb, however, this approach frequently originates increased morbidity. In addition, early amputation could have legal implications to take in account. Although most easily identified, skeletal injury is not the primary factor determining viability and functional potential; vascular, neurological and, specially, soft-tissue injuries play a much important role in predicting the survival and ultimate function of a mangled extremity. We report 4 cases treated in Hospital. We retrospectively calculated the Mangled Extremity Severity Score (MESS) punctuation, which is based on four clinical criteria. Efforts should be directed at establishing objective clinical guidelines to improve surgeon's experience when facing the difficult decision of amputation or salvage.
