RESUMEN: Este artículo provee algunos elementos de reflexión sobre las posibilidades y modalidades de la participación efectiva de los pueblos indígenas en la adecuación local del sistema de salud en el marco de las reformas neoliberales y multiculturales. Estas reformas han condicionado en los últimos 20 años el desarrollo del sistema de salud colombiano, pero también el rol de todos los actores. Los indígenas son concebidos tanto como agentes del sistema, como también usuarios. Los modelos de atención interculturales tendrán necesariamente que construirse desde un enfoque integral socioespacial del bienestar, dado que las concepciones indígenas de salud están íntimamente ligadas con la idea de “estar bien”, como un elemento adicional y resultante de las dinámicas socioambientales que incluyen especialmente “tener buen” territorio, acceder a medios de subsistencia, mantener el equilibrio social, entre otros. La participación social sigue siendo un tema central en dicho proceso de configuración local del sistema de salud. A pesar de la ampliación de los espacios y mecanismos para ella, las dinámicas económicas y políticas “globales” que imperan siguen condicionando su efectividad.

ABSTRACT: This article proposes elements to analyze the possibilities and modalities of an effective participation of indigenous peoples in the local adaptation of health systems within the framework of neoliberal and multicultural reforms. These reforms have conditioned in the last 20 years the development of the Colombian health system, but they have also conditioned the role played by all stakeholders. Indigenous peoples are conceived here both as agents of the system, and as users. It is stated that intercultural models of health care must be built from a necessary integral socio-spatial approach to wellbeing, since indigenous conceptions of health are inextricably linked with “being well” as a resulting element of the socio-environmental dynamics including being well and having a good territory, having livelihoods, maintaining the social order, among other factors. Social participation remains a central topic in the configuration of local health systems. Nevertheless, in spite of the increasing participation spaces and mechanisms, economic and political “global” dynamics prevail over the health system and the effectiveness of this social participation.
