Abstract

ABSTRACT: Objective: the aim of this paper is to offer clear recommendations for the clinical utilization of endomyocardial biopsy within the context of the heart failure syndrome, laying emphasis on evidence-based medicine through a systematic review of the literature. Methods: original research papers and meta-analyses were studied. Papers were found through Medline, Google Scholarship and Cochrane, by using the terms “endomyocardial”, “biopsy”, “myocarditis” and “cardiomyopathy”. Results: thirteen very heterogeneous articles were selected. Due to their heterogeneous nature, statistical measurements were not performed. In this paper the authors present an academic discussion of some of these articles. Conclusions: histopathologic study of cardiac tissue has become a useful tool in the workup of patients with congestive heart failure and dilated cardiomyopathy. The incidence of complications varies from 1 to 6%, with a mortality rate of 0.4% according to the clinical series studied. Mortality is decisively in fluenced by the operator’s experience. In order to optimize its diagnostic possibilities, biopsy should be accompanied by complete anatomopathological analysis, including conventional,special and immunohistochemical stains, as well as new techniques such as polymerase chain reaction (PCR), and reverse transcriptase-PCR analysis for most cardiotropic viruses and the expression of HLA antigens. Endomyocardial biopsy is a useful tool in the follow-up of the heart transplant patient, in order to diagnose acute rejection. It is also useful in the follow-up of patients with drug toxicity issues, and in order to diagnose patients with systemic or in filtrative diseases, new onset congestive heart failure and other conditions

RESUMEN: Objetivo: revisar las recomendaciones para la utilización clínica de la biopsia endomiocárdica en el contexto del síndrome de falla cardiaca, con énfasis en la medicina basada en la evidencia a través de una búsqueda sistemática de la literatura. Metodología: se analizaron trabajos de investigación originales y metaanálisis en Medline, Google scholarship y Cochrane con los siguientes términos: “endomyocardial”, “biopsy” cruzados con “myocarditis” y “cardiomyopathy”. Resultados: se seleccionaron 13 artículos, dada la heterogeneidad de las publicaciones se presenta solo la discusión académica de algunos de ellos. Conclusiones: el estudio histopatológico del tejido cardiaco se ha convertido en una herramienta útil en la evaluación de algunos pacientes con síndrome de falla cardiaca y cardiopatía dilatada. La incidencia de complicaciones varía del 1 al 6%, con mortalidad del 0.4% de acuerdo a la serie revisada e influenciada en gran medida por la experiencia del operador. Para optimizar el rendimiento diagnostico se debe realizar un análisis anatomopatológico completo que incluya tinciones convencionales y especiales, inmunohistoquimica y técnicas nuevas como reacción de polimerasa en cadena (PCR), análisis de transcriptasa reversa por PCR para la mayoría de los virus cardiotrópicos y la expresión de antígenos de HLA. La biopsia endomiocardica es una herramienta útil en el seguimiento del paciente trasplantado, para diagnostico de rechazo, en el seguimiento de los pacientes con toxicidad por antracíclicos, para establecer diagnostico en pacientes con sospecha de enfermedades sistémicas y/o infiltrativas y en la falla cardiaca de reciente comienzo entre otras.
