Abstract

RESUMEN: El síndrome de Wiskott-Aldrich es una compleja enfermedad de origen genético que compromete la función del sistema inmune y la coagulación; la mortalidad es elevada en la primera década de la vida a menos que se dé una corrección definitiva al defecto de base. Debido a que las alteraciones se presentan exclusivamente en células derivadas de la médula ósea, el tratamiento ideal es el trasplante de este órgano. Sin embargo, no siempre se puede conseguir un donante apto para este procedimiento, y deben surgir otras opciones de manejo que incrementen la sobrevida y prevengan el desarrollo de las secuelas más frecuentes. El tratamiento combinado con esplenectomía, antibióticos profilácticos y gamaglobulina humana venosa permite lograr estos objetivos, además de disminuir los costos de atención y mejorar notablemente la calidad de vida del paciente y su familia.

ABSTRACT: The Wiskott-Aldrich syndrome is a complex disease of genetic cause which produces alterations in the function of immune and clotting systems. If a definitive therapy that fix the underlying defect is not done, mortality is high in the first decade of life. Due to the fact that alterations are present exclusively in those cells produced in bone marrow, the most efficient therapy is bone marrow transplantation. However, sometimes it is not possible to get an adequate donor to do this procedure, and other options are needed to improve the life expectancy and to avoid deleterious consequences. The combined treatment with splenectomy, antibiotic profilaxis and intravenous gammaglobulin allow to get these objectives; furthermore, it decreases the management costs and improve the patient's quality of life.
