Abstract

La enfermedad renal es una condición clínica heterogénea que afecta la funcionalidad de los riñones y se caracteriza por su naturaleza silenciosa. El fracaso renal agudo (FRA) es característico del paciente hospitalizado y tiene una alta mortalidad (50%), lo que supone 2 millones de muertes al año. Se prevé que la enfermedad renal crónica (ERC) se convierta en la quinta causa mundial de muerte para 2040. La ERC aumenta el riesgo de sufrir FRA, y a su vez, el FRA puede acelerar la progresión de la ERC. Actualmente, no existe un tratamiento efectivo para el FRA más allá de la sustitución de la función renal mediante diálisis. Por otro lado, CD74 es una glicoproteína transmembrana no polimórfica de tipo II que regula el tráfico de proteínas y participa en eventos moleculares involucrados en el FRA y la ERC. Sin embargo, su papel específico en el riñón sigue siendo desconocido. Los modelos experimentales revelaron un incremento en la expresión de CD74 en los túbulos renales proximales durante el FRA. La deficiencia de CD74 agravó el desarrollo del FRA, tanto en condiciones nefrotóxicas como isquémicas, manifestándose en un aumento de la mortalidad, una respuesta inflamatoria más aguda y una disminución de los niveles de expresión de Klotho. Además, en células tubulares murinas, se observó un aumento de la expresión de CD74 y su translocación al núcleo en respuesta a estímulos proinflamatorios. Este aumento de expresión tubular de CD74 también se confirmó en pacientes con FRA, respaldando la relevancia traslacional de las observaciones preclínicas. En modelos experimentales de ERC, se detectó un aumento en la expresión de CD74 localizado en los túbulos renales. La deficiencia de CD74 aceleró la fibrosis y promovió un desarrollo más severo de la ERC al intensificar respuestas deletéreas como la inflamación y la pérdida de expresión de Klotho. En consecuencia, la expresión de CD74 parece formar parte de la respuesta compensatoria y protectora del riñón desencadenada por la inflamación local durante el daño renal, y parece estar implicada en el desarrollo de la fibrosis

Kidney disease is a heterogeneous clinical condition that affects the functionality of the kidneys and is characterized by its silent nature. Acute kidney injury (AKI) is prevalent among critically hospitalized patients and carries a high mortality rate (50%), resulting in 2 million deaths per year. By 2040, chronic kidney disease (CKD) is expected to become the fifth leading global cause of death. Notably, CKD increases the risk of AKI, and in turn, AKI can accelerate CKD progression. Currently, there is no effective treatment for AKI beyond replacing renal function through dialysis. Moreover, CD74, is a non-polymorphic type II transmembrane glycoprotein that regulates protein trafficking and participates in molecular events involved in AKI and CKD. However, the role of CD74 in the kidney remains unknown. Increased CD74 protein was localized to proximal tubular cells in AKI. Notably, in human AKI cases, CD74 was found present in proximal tubular cells in kidney sections, supporting the translational relevance of preclinical observations. CD74 deficiency increased the severity of murine ischemic- or folate-induced AKI as characterized by higher mortality, lower kidney function, more severe kidney inflammation, and Klotho downregulation. Additionally, in murine tubular cells, increased CD74 expression and its translocation to the nucleus were observed in response to pro-inflammatory stimuli. Regarding CKD experimental models, CD74 protein levels were increased in CKD, primarily localized within tubular cells. CD74 deficiency accentuated the severity of unilateral ureteral obstruction (UUO) and adenine-induced murine models by accelerating fibrosis and enhancing deleterious responses such as inflammation and Klotho loss. Therefore, CD74 expression might be part of the kidney´s compensatory and protective response triggered by local inflammation during renal damage and seems to be involved in fibrosis development
