Abstract

Introducción: La Diabetes Mellitus Tipo 1 es una enfermedad autoinmune que ha sufrido un gran incremento en su prevalencia en los últimos años. El tratamiento de la diabetes ha evolucionado a lo largo del tiempo, así como los avances tecnológicos que se han ido sucediendo. Uno de los ejes del tratamiento de la DM1 es la automonitorización continua de glucosa, la cual también ha mejorado con la aparición de las nuevas tecnologías. Estas han ido cobrando fuerza entre los pacientes con DM1 con el uso de sensores de MCG transcutáneos, actualmente los más utilizados, o con el sensor implantable. Objetivos: Analizar el sensor implantable de monitorización continua de glucosa como nuevo método de monitorización de glucosa frente a los sensores transcutáneos. Metodología: Se ha realizado una revisión narrativa, entre 2015 y febrero de 2023, en las bases de datos de Pubmed, Cinahl, Web of Science y Cochrane. Resultados: Se revisaron veintiséis artículos, cuyo contenido se ha agrupado de forma que, por un lado, se analizaron aquellos artículos que abordaban la efectividad y seguridad del sensor implantable, a través de la MARD, efectos sobre la HbA1c y el tiempo en rango, efectos adversos, adherencia terapéutica e impacto psicológico, con resultados favorecedores para este sensor. También se compararon las ventajas y desventajas con respecto a las funciones del sensor implantable y de otros dispositivos de MCG; en este caso, los resultados obtenidos fueron diversos, ya que ambos métodos presentaban tanto ventajas como barreras. Por último, se revisaron los artículos que desarrollan la función de la enfermera a través de las intervenciones asociadas al sensor implantable, identificándola como figura esencial para el control glucémico con este dispositivo. Conclusiones: El sensor implantable de MCG es un método efectivo y seguro que presenta ciertas ventajas frente a los sensores transcutáneos. Es necesario seguir investigando las funciones de este sensor para adquirir mejoras y poder acoplarlo a las bombas de infusión continua de insulina. Además, se considera importante la formación de los profesionales en estas tecnologías, así como la promoción de su uso.

Introduction: Type 1 Diabetes Mellitus is an autoimmune disease that has experienced a great increase in its prevalence in recent years. The treatment of diabetes has evolved over time, as well as the technological advances that have been taking place. One of the axes of DM1 treatment is continuous glucose self-monitoring, which has also improved with the apparition of new technologies. These ones have been gaining strength among patients with DM1 with the use of transcutaneous CGM sensors, currently the most widely used, or with the implantable sensor. Objectives: Analyze the Implantable continuous glucose monitoring system as a new glucose monitoring method compared to transcutaneous sensors. Methodology: A narrative review has been carried out, between 2015 and February 2023, in Pubmed, Cinahl, Web of Science and Cochrane databases. Results: Twenty six articles were reviewed, whose content has been grouped in such a way that, on the one hand, those articles that addressed the efficacy and safety of the implantable sensor, were analyzed through the MARD, effects on HbA1c and time in range, adverse effects, therapeutic adherence and psychological impact, with favorable results for this sensor. The advantages and disadvantages were also compared regarding the functions of the implantable sensor and other CGM devices; in this case, the results obtained were diverse, since both methods presented advantages and barriers. Finally, the articles that develop the role of the nurse through the interventions associated with the implantable sensor were reviewed, identifying it as an essential figure for glycemic control with this device. Conclusions: The implantable CGM sensor is an effective and safe method that has certain advantages over transcutaneous sensors. It is necessary to continue investigating the functions of this sensor to acquire improvements and be able to couple it to continuous insulin infusion pumps. In addition, the training of professionals in these technologies is considered important, as well as the promotion of their use.
