Bypass extra- intracraneal en el tratamiento de los aneurismas cerebrales complejos

Abstract

Presentamos los resultados de nuestra experiencia, pionera en España, en uso del bypass extra-intracraneal en el tratamiento de los aneurismas complejos que no pueden ser tratados mediante clipaje quirúrgico o embolización. Describimos nuestra clasificación funcional de los aneurismas en los que es útil el bypass extra-intracraneal y establecemos en cada tipo de aneurisma un protocolo individualizado de tratamiento. Proponemos una nueva clasificación de los aneurismas fusiformes. Los dividimos en dos grupos en función de la presencia o no de vasos perforantes en su interior. Definimos el tratamiento más adecuado en cada uno de los grupos. Sistematizamos el procedimiento quirúrgico y reducimos la operación a siete pasos. Explicamos en detalle el porqué de cada detalle de la cirugía para optimizar los resultados y reducir las complicaciones. Introducimos un nuevo protocolo de actuación en el manejo intraoperatorio de las oclusiones del bypass. Se propone un esquema para la planificación del bypass, basándonos en los estudios de arteriografía preoperatorios sin necesidad de mediciones de flujo. Esto nos permitirá una elección segura de los vasos donante, receptor y de los injertos. Finalmente, desde la introducción en los últimos años de instrumentos para mediciones directas del flujo, estudiamos el flujo a través del bypass Definiendo los conceptos de bypass de bajo y de alto flujo.
