Bilirrubina covalente : estudio en las enfermedades hepatobiliares

Abstract

Una cuarta bilirrubina ha sido identificada, llamada bilirrubinacovalente, delta o biliproteina, la cual esta constituida por bilirrubina conjugada unida por enlace probablemente covalente a la albumina. para estudiar su importancia clínica hemos determinado la bilirrubina covalente en 499 muestras de 226 individuos mediante cromatografia con resina intercambiadora de aniones, método ideado por seligson. en sujetos normales, en pacientes con hiperbilirrubinemia indirecta excluidos aquellos con síndrome de gilbert y en pacientes con colostasis disociada sin hiperbilirrubinemia reciente no detectamos la bilirrubina covalente en suero en la mayoría de ellos, y en los que se detecto fue en mínimas concentraciones (0.15 mlgr/100). en 7 pacientes con síndrome de gilbert detectamos la biliproteina aumentada en 4. los pacientes con hiperbilirrubinemia directa presentaban aumento de la bilirrubina covalente serica, no existiendo diferencias entre aquellos con patología intra o extrahepatica; la proporción entre la bilirrubina covalente con respecto a la bilirrubina directa tuvo interés pronostico siendo significativamente mas alta en aquellos que evolucionaron favorablemente, dicha proporción fue también significativamente en la fase resolutiva de la ictericia. en la bilis de sujetos normales la biliproteina fue menor de 0.15 mlgr/100, lo que contrasta con el aumento de la misma en la bilis de pacientes con colostasis disociada o con hiperbilirrubinemia directa. en la orina de pacientes con hiperbilirrubinemia directa sin proteinuria la biliproteina fue menor de 0.15 mlgr/100). en liquido ascitico detectamos bilirrubina covalente que mostró una alta correlación con la albumina en dicho liquido.
