Abstract

El cáncer de mama es uno de los tumores más frecuentemente diagnosticados a nivel global, y tiene un importante impacto sanitario en la población de mujeres debido a su gran incidencia y tasa de mortalidad asociada. Entre los diferentes subtipos que existen, atendiendo a la presencia, o no, de los receptores de estrógeno, progesterona y HER2 (Human Epidermal growth factor Receptor 2), encontramos el cáncer de mama triple negativo (TNBC), denominado así por la ausencia de estos tres receptores. Este subtipo es el más agresivo de cáncer de mama, debido en gran medida a su heterogeneidad celular, favorecida además por procesos como la transición epitelio mesenquimal (EMT), y a la capacidad metastásica junto a la resistencia a la radio/quimioterapia debida a la población de células madre tumorales (CSCs). Todas estas propiedades tumorogénicas están reguladas por diferentes vías de señalización, como es el caso de la ruta de TGFβ. Estudios anteriores de nuestro grupo revelaron que al estimular con TGFβ1 las células mamarias MCF10A, se observaba una sobreexpresión del gen ATF4 (Activating Transcription Factor 4), sin que hubiese estrés celular. Al contrastar la expresión de este gen en las bases de datos de pacientes de cáncer de mama, vimos que se podía asociar altos niveles de expresión con un peor pronóstico. Bajo estas premisas, investigamos si ATF4 puede regular o mediar la agresividad inducida por TGFβ en TNBC, y afectar de este modo a la supervivencia de las pacientes. Este trabajo es el primero en demostrar el potencial de ATF4 como un nuevo biomarcador pronóstico y diana terapéutica frente al cáncer de mama triple negativo.

Breast cancer is one of the most frequently diagnosed tumors and has a significant health impact on the female population due to its high incidence and associated mortality rate. Among the different subtypes that exist, depending on the presence or absence of hormonal receptors for estrogen, progesterone and HER2 (Human Epidermal growth factor Receptor 2), we find the triple negative breast cancer subtype (TNBC), that lacks those three receptors. This subtype is the most aggressive of all breast cancers, due to cellular heterogeneity, favored by processes such as mesenchymal epithelial transition (EMT), its metastatic capacity along with resistance to radio / chemotherapy, by the population of cancer stem cells (CSCs). All these tumorigenic properties are regulated by different signaling pathways, as the TGFβ pathway. Our previous studies of our group revealed that upon stimulating the MCF10A mammary cell line with TGFβ1, an overexpression of the ATF4 gene (Activating Transcription Factor 4) was observed, in absence of cellular stress. By contrasting the expression of this gene in the databases of breast cancer patients, we observed that high levels of expression could be associated with a worse prognosis. Under these premises, we investigated whether ATF4 can regulate or mediate TGFβ-induced aggressiveness in TNBC, and thus affect patient survival. This work is the first to demonstrate the potential of ATF4 as a new prognostic biomarker and therapeutic target against triple negative breast cancer.
