Importância terapêutica dos hemoderivados : perspectivas actuais e futuras

Abstract

O plasma humano contém muitas proteínas que são de grande importância médica. Após o fraccionamento de pools de plasma, proveniente de milhares de dádivas, obtém-se os medicamentos derivados do plasma, também designados por hemoderivados. Estes medicamentos são cruciais na profilaxia e tratamento de doentes com deficiências imunológicas, doenças autoimunes e inflamatórias, distúrbios hemorrágicos e uma variedade de deficiências congénitas. Todo o processo de produção de hemoderivados leva a que no final, estes produtos reúnam três premissas essenciais: eficácia terapêutica, tolerância clínica e segurança viral. Devido a possíveis problemas relacionados com transmissão de doenças infecciosas, a prescrição, dispensa e administração é muito rigorosa, sendo controlada por normas legais em todo o mundo, permitindo uma rastreabilidade destes produtos. Esta monografia aborda a importância terapêutica de cada um dos hemoderivados utilizados em Portugal, explicando o seu mecanismo de acção e caracterizando sumariamente a patologia envolvida. Os medicamentos derivados do plasma incluem a albumina humana, as proteínas de coagulação, as proteínas de anti-coagulação, a imunoglobulina humana e as imunoglobulinas específicas. A utilização de hemoderivados é cada vez maior e uma vez que o plasma é um bem escasso, abordaremos as diferentes técnicas de produção de medicamentos não derivados do plasma que tenham a mesma função.

Human plasma contains numerous proteins that are of great medical importance. After fractionation of plasma pools, from thousands of donations, plasma-derived medicinal products are obtained. These drugs are crucial in the prophylaxis and treatment of patients with immune deficiencies, autoimmune and inflammatory diseases, bleeding disorders, and a variety of congenital deficiency disorder. The entire process of producing plasma-derived medicinal products leads to the fact that, in the end, these products meet three essential premises: therapeutic efficacy, clinical tolerance and viral safety. Due to potential problems related to the transmission of infectious diseases, the prescription, dispensing, and administration is extremely strict, being controlled by legal standards around the world, allowing traceability of these products. This monograph addresses the therapeutic importance of each of the plasma-derived medicinal products used in Portugal, explaining their mechanism of action, and briefly characterizing the pathology involved. Plasma-derived medicinal products include human albumin, coagulation proteins, anticoagulation proteins, human immunoglobulin, and specific immunoglobulins. The use of plasma-derived medicinal products is increasing, and since plasma is a scarce commodity, we will discuss the different techniques to produce non-plasma-derived drugs that have the same function.
